We provide resources and information pertaining to in home care, caregivers, home health, hospice, dementia, board and care homes, assisted livings, independent living, memory care and skilled nursing. we also provide information regarding veterans services, community resources and support. As a Care Manager we :
*Advocate on your behalf with your doctors and hospitals, asking those questions most people do not know how to ask

*Educate you on what is happening with your condition/diagnosis

*Attend doctor’s appointments with you

* Coordinate Care - Nurses, Physical Therapy, Non Medical Care, Doctor's Appointments etc.

*Provide clear communication to and from your doctors, family members, and anyone playing a role in your care, to ensure everyone is on the "same page"

*Provide support and navigation with new diagnoses, complex medical scenarios, dementia diagnoses, emergency room visits, Adult Protective Services involvement

*Assist you in understanding all of your care options and selecting services or placement that is right for you (assisted living, hospice, memory care, short term rehab, physical therapy, counseling, board and care etc.)

*Research all of your treatment options, educating you and your family, ensuring that you consult with the most appropriate doctors, so you make the best choice for your health.

* I do not make healthcare choices for you or your family.
* I do not diagnose ailments, order treatments or replace the physician in any way.
*As an Advocate I am a complement to the hospital staff, not a replacement for it.
* I do not replace the Social Worker, Physical Therapist or Nutritionist or any other medical professional involved in your care. I work directly FOR YOU and will then collaborate with all members of your healthcare team to ensure your best possible treatment for the greatest outcomes.

Amari Stewart-Raney, 11 years old, 5’4”, 130 pounds, with black cornrow-style hair and brown eyes. He was wearing a gray hoodie with a black cat on the front, black pants, blue Crocs, and 49ers socks. Amari was last seen on foot near the 4300 block of 46th Avenue in South Sacramento.

He is considered at risk due to his age and having autism.

Your Elder Care Specialist is a one-stop resource for finding solutions to all of your senior care needs. We are a free service, helping seniors and their families find appropriate care options (in home care, home health, hospice services) and senior facility placement (independent living, assisted living, board and care and memory care).

Rebecca Johnson, founder, graduated from CSU Sacramento with Bachelor's Degree in Social Work, and has over 10 years of working with the elderly, and over 20 years of case management and advocacy work with patients and their families.

She prides herself on helping families during times of crisis, as most seniors aren’t ready for care, until “something” happens. She helps with hospital discharges, arranging for care, navigating insurance and paperwork, finding resources and following up on your progress long term. She is a registered Mental Health Rehabilitation Specialist, professional patient advocate and specializes in working with complex care.
