Essential Functions:
*Ensures timeliness of documentation and that documentation accurately reflects services provided.

*Initial and ongoing comprehensive assessments of home health patient’s needs, including OASIS assessments at appropriate time points.

*Develop/initiate/revise the plan of care as necessary and initiate appropriate preventive and rehabilitative nursing

*Coordinate patient care, make necessary referrals, report to physician/other staff members changes in patient needs, evaluate outcomes of care and plan for discharge from service.

Job Qualifications :
Current state license as a Registered Nurse, current Texas driver license, current CPR,
Reliable Transportation. Valid and current auto liability insurance .

Medical record order specialist has primary responsibility and accountability for maintaining accurate patient records for billing purposes, supply inventory and other related clerical duties. Keep accurate reports on pending physician order and or plan of cares to ensure timely billing and reports to billing Manager, Office Manager or Director of Nursing.

2 Files all elements of patient records on a daily basis.
3 Date stamps all clinical notes on a daily basis.
4 Prepares Medical Record for retrieval and follows process to locate, sign out retrieve and deliver medical records as requested.
5 Keep accurate reports on pending physician order and or plan of cares to ensure timely billing and reports to billing Manager, Office Manager or Director of Nursing
6 Receives and routes daily incoming mail.
7 Maintains current log of interim orders and plans of care (POC) to verify mail and return date to comply with state and federal regulations.
8 Maintains adequate inventory of medical supplies/items needed to provide appropriate patient care, notifies the clinical supervisor when inventory needs to be restocked.
9 Maintains adequate supply of patient clinical record elements and stock of pre-made clinical records available for clinicians.
10 Answers telephone lines promptly and efficiently. Responds to message content appropriately, as instructed by the Clinical Director/Information Systems Supervisor. Records telephone calls onto call log.
11 Responsible for having excellent time management skills for field job duties.
12 Keeps daily logs of mileage used and submit for payroll on a weekly basis.
13 Special projects and other related duties as assigned by the Clinical Director/ Office Manager / Billing Manager.
14 Files medical records on a daily basis in patient chart.
15 Establishes and maintains positive working relationships, with current and potential referral and payer sources.

Licensed vocational nurse responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatment, and working collaboratively what’s the members of the team to help me positive patient care outcomes.

Essential job functions/responsibilities:

- provides direct patient care as defined in state nurse practice act
- provides accurate and timely documentation consistent with the plan of care. Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- participates in coordination of home health services, appropriately reporting that identify needs for other disciplines to the clinician and/or Director of clinical services.
-  uses equipment and supplies effectively and efficiently.
- Able To do home visits in the El Paso area

Position qualifications:
-Graduate of an accredited practical nurse or vocational nursing program.
- has two years nursing experience.
- currently licensed as a licensed practical nurse or license vocational nurse in the state of Texas.
- complies with excepted professional standards and practice.
- Demonstrates good verbal and written communication skills.
- Must be a licensed driver with an automobile that is insured in accordance with the state of Texas and home Sweet Home unlimited requirements and in a good working order.
