La Familia is seeking a motivated individual to coordinate patient/client services for the in-home provider care program (also known as PAS or PHC services) by providing a channel through which patients/clients or caregivers can seek solutions to problems, concerns and/or unmet needs.

Responsibilities will include:

Answering incoming calls regarding patient/client services.

Processing patient information our agency database (Vesta)

Supervising attendant schedules for patient services, scheduling orientations, and supervisory visits.

Assisting with finding attendants and/or service coverage for patients.

Communicating with state caseworkers and insurances to coordinate services.

Pay Rate: $11.00 to $13.00 an hour.

La Familia is looking for attendants who are willing to work on Saturdays and Sundays.

Attendants are responsible for providing care and assisting our clients with their needs within the client's home. Attendants help fulfill the agency's mission by ensuring that authorized tasks are rendered to our clients under the Provider Services guidelines.

Weekend attendants would have a schedule ranging from 10-20 hours for $10 -$10.50 an hour.

La Familia is looking for CNAs to join our team!

A CNA's main job function will be to provide our patient's physical support with daily activities and hygiene including bathing, dressing, getting out of bed, using the toilet, walking, standing, exercising, turning and repositioning bedbound patients. A CNA will also be responsible for recording vital signs and completing necessary visit documentation as directed by our RNs.

$100 sign on bonus after 90 days of employment!

La Familia is looking for a full time PAS coordinator. A PAS coordinator's primary focus is providing quality care to our clients. The main responsibility of a coordinator is to match the client's needs with an attendant who has the ability and skills to provide care tailored to the each of our client's needs.

As a PAS coordinator will also be responsible for submitting appropriate documentation into the system as well as the caseworker for each client as indicated.

PAS coordinators are expected to have open communication between the client's, attendants, as well as fellow PAS team members to ensure quality care is provided.

Bilingual applicants preferred (English/Spanish).
Basic computer skills required.
