Rated 5 stars in Quality Measures by Medicare, our facility makes recovery feel smooth and personal. Residents benefit from a full range of therapies, wound care, peritoneal dialysis and rehabilitation. Those in memory care enjoy our secured Garden unit, and the REACH program (Risk Assessment, Education and Cardiac Health), led by in-house electrophysiologist Dr. Paul Nguyen, provides advanced cardiac support and education on-site.

We take pride in knowing every resident by name, partnering closely with their loved ones and care teams to help each individual meet their goals and return home with confidence.

Every ADL milestone begins with our rehab team! 🏋️‍♂️🩺 During Rehabilitation Awareness Week, we recognized their work personalizing therapy sessions and supporting each resident’s progress. Their expertise guides recovery and helps residents return home with confidence. Thank you for the excellence you bring every day!

📸 As part of their celebration, the team spent an afternoon giving back at Feed My Starving Children - Schaumburg. Together, they packaged enough food to feed 65 kids every day for a full year!
