BREAKING NEWS..... Dr. Jenkins will open tomorrow from 10am-12pm for sports physicals only! $25
Here are the guidelines.... arrive and form a line on the sidewalk with 6ft between patients. If possible, click the link below and print your physical form. Have it filled out when you arrive (fill out the front and top of the back). One athlete and one parent will be allowed in the office at a time. If you need a physical form, one will be provided. Your patience is appreciated!!!! We want to help parents and athletes get back into action, but SAFETY is our #1 priority!
This is first-come first-serve....... During regular business hours, you will need an appointment.
