Meet Elizabeth✨

Elizabeth is our newest licensed aesthetician and lead cosmetic coordinator. With a passion for skincare, aesthetics, and patient care, she brings warmth, knowledge, and a personalized touch to every appointment.

Elizabeth specializes in customizing skincare treatments and guiding patients through their aesthetic journeys. Her goal is to ensure every client feels confident, informed, and supported-from the first consultation to post-treatment follow-up.

Meet Colleen✨

Colleen provides aesthetic treatments in our Crystal Lake office and is known for her warm approach, attention to detail, and commitment to helping patients feel confident in their skin.

She began her career in spa-based skincare performing facials, waxing, and chemical peels before transitioning into the medical side of aesthetics to offer more advanced, results-driven treatments.

Colleen specializes in customized skincare plans and advanced treatments that promote long-term skin health and enhance natural beauty. She’s especially passionate about educating teens and younger patients on skincare and self-care, helping them build healthy habits and confidence early.

Meet Jenn✨

Jenn has been an aesthetician for 20 years and has been with Mohs Surgery & Dermatology since we opened our doors in 2008! She’s played a major role in growing the aesthetic side of our practice - from building our menu of treatments to helping select many of our devices and products we offer today.

Our Elgin office is home to most of our aesthetic devices and lasers with providers from all of our locations referring their patients to Jenn. She’ll tell you Sciton BBL and Halo are her favorites.. she’s performed over 4,000 laser treatments! Jenn is known for her expertise, warmth and results driven approach.

Meet Jacob Klaustermeier, PA-C ✨

Jake Klaustermeier, PA-C, has been part of the dermatology field since 2007 and plays an important role at Mohs Surgery & Dermatology Center. Over his career, Jake has received specialized training in Mohs surgery, which has fueled his passion for creating customized treatment plans that emphasize prevention and early detection and treatment of skin cancer. He prioritizes ongoing education and stays up to date on the latest advancements in both medical and surgical dermatology.

Before becoming a PA, Jake served in the U.S. Army Reserves as a Civil Affairs Specialist and was deployed to Afghanistan in 2003. Upon his return from service, he completed his master’s degree in physician assistant studies at Rosalind Franklin University, Illinois.

When he’s not in the office, Jake is an avid runner who enjoys being outdoors and spending time with his Frenchie, Gunther.🏃🏻

With 24 years of experience in dermatology, Tamara is a trusted expert in medical and cosmetic dermatology and a proud member of the Mohs Surgery & Dermatology Center team. She has served as adjunct faculty at her alma mater, Midwestern University, and is a respected leader in the field, having also served as president of the Illinois Society of Dermatology Physician Assistants.

Tamara believes in treating every patient like family-listening closely, addressing their concerns, and providing thoughtful, quality care. Her goal is to help each person feel confident and comfortable in their skin.

Meet Chelsy Conrad, MMS, PA-C ✨

Chelsy has been with the practice for 11 years and has built lasting relationships with her patients - some of whom she now sees more often than her extended family. She has a particular interest in skin cancer screenings and treatment, and enjoys helping her cosmetic patients age gracefully (or at least convincingly). A board-certified physician assistant with extensive experience in treating medical skin conditions like acne, rosacea, eczema, and psoriasis, Chelsy regularly performs full-body skin exams and surgical treatments.

Chelsy is also one of our cosmetic injection gurus, loved by patients for her honest approach, artistic eye, and ability to enhance natural beauty without overdoing it. She’s trained in injectable fillers, toxins, microneedling, PRP, and custom skincare regimens to help patients look and feel their best.

When she’s not at work, Chelsy and her husband are lucky to raise three small kids and one overly attached Goldendoodle, thriving and barely surviving, all at once. Most weekends, you’ll find them outdoors, soaking up sunshine (with SPF 50 and hats), chaos, and the occasional moment of peace.🌿

Meet Ashley Jansen, PA-C ✨

With 17 years of experience as a Physician Assistant in dermatology, Ashley brings both expertise and compassion to every patient encounter. She is passionate about caring for patients of all ages and specializes in medical and surgical dermatology, including PRP (platelet-rich plasma) therapy for hair loss. She creates individualized treatment plans tailored to each person’s unique needs and goals. Ashley believes in meaningful patient interactions-striving to ensure that everyone feels safe, supported, and confident in their skin. Dedicated to patient education, she takes pride in helping individuals understand their skin health while guiding them through both preventive and corrective care.

Meet Kathy Guerra, PA-C ✨

With a passion for patient education and a deep background in medical and cosmetic dermatology, Kathy is a valued member of the Mohs Surgery & Dermatology Center team in Elgin, Illinois.

Practicing since 2010, she brings experience ranging from caring for underserved communities in Los Angeles to treating cosmetic clients on Rodeo Drive in Beverly Hills. Kathy has a special interest in acne, warts, and skin cancer care, as well as expertise in Botox® and dermal fillers for natural, refreshed results. She also shares her knowledge with peers- speaking annually at the National AAPA Conference to help educate other physician assistants across the country.

At home, Kathy is a proud mom of three, a football coach’s wife, and dog mom to a large German Shepherd.
