As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home.

Care Guide Partners PACE helps adults 55+ keep living at home. We bring care to you in one plan and one team. Doctors and nurses. Occupational therapy and physical therapy. Medications and meals. Rides to appointments. Support for caregivers.

Our team shows up in real ways every day. The scheduler who knows your routine. The Occupational Therapist who helps you practice safe ways to dress and move. The CNA who helps with bathing and notices small changes. The driver who safely transports you to and from the center. The nurse who checks in because you were a little tired yesterday.
