Life Care Center of Elyria Near the forks of the Black River, Life Care Center of Elyria, Ohio, is less than 3 miles from Elyria Medical Center.

Our nursing home is not just a place for 24-hour skilled nursing care for long-term residents but also a place for short-term inpatient and outpatient rehabilitation with physical, occupational and speech therapies. We're so proud of Robert for completing his rehabilitation program with us! 🎉

Robert sustained multiple falls at home resulting in progressive hip pain and muscle tear. This impacted his ability to walk, get around and take care of himself independently. His daughter-in-law is a physical therapist at Life Care Center of Elyria and recommended he travel here for effective rehab with the most caring people. ☺️

Several challenges rose before Robert, including decreased strength, balance, and functional mobility. He had significant mobility limitations and a high risk of falls. Our physical and occupational therapy team worked together to create a treatment plan, and Robert made great progress! 👍

When describing his overall experience, Robert states, "Everyone here is patient and respectful. People great each other and the patients by their first name. That is a classic sign of a good place. Also, therapists are great, the meals are generous and tasty, and I like the therapy dog!" 🐶

Robert returned home able to complete his self-care tasks, transfers and walking independently. He and his wife are transitioning next month to The AbbeWood, our partnering senior living campus next door.

As he continues his journey, he will maintain consistency in his rehab through outpatient therapy with the same therapists he had at Life Care. Congratulations to Robert!

Congratulations to Mr. Alvie for recently completing his rehabilitation with us and returning home independently! 🎉

Alvie came to us after a surgery that impacted his ability to walk, transfer or take care of himself. Our physical and occupational therapists completed comprehensive evaluations and put together a dynamic, integrated treatment approach to improve his ability to complete his daily routine independently. Alvie made great progress!

🗨️ "I have nothing but great things to say about my rehab stay," Alvie shared. "Everyone has been so nice to me. I received excellent nursing care for my medical needs, and the therapists have been top-notch! I have been to a lot of hospitals (facilities), but this one gets an A+, 100%!"

Congratulations to Alvie for working so hard with our staff! Alvie credits team members for their support, and not just his therapists! He also wanted to include thanks to the dietary, housekeeping, nursing and CNA staff for working together to set him up for success. 💛
