Life Care Center of Elyria Near the forks of the Black River, Life Care Center of Elyria, Ohio, is less than 3 miles from Elyria Medical Center.

Our nursing home is not just a place for 24-hour skilled nursing care for long-term residents but also a place for short-term inpatient and outpatient rehabilitation with physical, occupational and speech therapies. Congratulations to Jeffery, who recently completed his rehabilitation journey with Life Care Center of Elyria after surgery! 🎉

Jeffery first came to us as a short-term inpatient, staying on-site, and then chose to continue his recovery with us as an outpatient, living and home and coming in a few days a week to continue physical therapy. He regained strength and balance, as well as endurance and confidence! It even helped resolve his lower back pain. 😁

Congratulations to Constance, who recently completed her rehabilitation with us and returned to her apartment at The AbbeWood Assisted Living next door! 👏

When she came to us after a hospital stay, she was weak, required supplemental oxygen, had poor balance, and needed assistance for her self-care tasks. Our physical and occupational therapists implemented a treatment plan just for her. 😊

🗨️ "I don't like to exercise," Constance said. "The therapists were kind, addressed my goals, and were able to adjust to my needs. I came here because I understand that Life Care is the best. I was born and raised here in Elyria and remember when this place was built. It was a big deal!"

Constance was able to regain her independence in mobility and self-care, and get off the supplemental oxygen. She's back at her apartment to enjoy her friends and the holidays! 🎄

🎉 Congratulations to Charles, who recently completed his rehabilitation program with us!

He came to us with weakness, impaired balance, and limited ability to walk and take care of his daily self-care tasks. Our physical and occupational therapy teams worked with him on his unique plan of care, and he returned home able to complete his functional mobility and self-care tasks independently! 👍
