Single leg balance

Why is single-leg balance so important? Functional tasks we do every day such as walking, getting into a tub, putting on your pants, and going up/down stairs all require adequate single-leg balance. Here’s a quick single-leg balance screen you can perform at home and here are the average age-related scores as discussed in the video:

Age 18-39: 43.3 seconds with eyes open; 9.4 seconds with eyes closed

Age 40-49: 40.3 seconds with eyes open; 7.3 seconds with eyes closed

Age 50-59: 37 seconds with eyes open; 4.8 seconds with eyes closed

Age 60-69: 26.9 seconds with eyes open; 2.8 seconds with eyes closed

Age 70-79: 15 seconds with eyes open; 2 seconds with eyes closed

Age 80-99: 6.2 seconds with eyes open; 1.3 seconds with eyes closed

What is Persistent postural‐perceptual dizziness (PPPD or 3PD)? 3PD is a chronic balance disorder that is characterized by unsteadiness or dizziness that is often triggered by changes in position or visual stimulation. Symptoms are present on most days for at least three months. Although the exact mechanism is unknown, 3PD is understood as a miscommunication between the 3 systems we use for balance (vision, somatosensation, and vestibular system). Mal de Débarquement originates from a French term dating back to 1575 that translates to “sickness of disembarkment.” Mal de Débarquement Syndrome (MdDS) is the perception of self-motion after exposure to passive motion that persists for more than 3 days and up to several years. Clinically, patients experience a rocking, bobbing, or swaying sensation which is often accompanied by unsteadiness and disequilibrium that occurs persistently after stopping the passive motion stimulus. Most cases stem from sea travel (73%) followed by air travel (41% or less) and land travel (16% or less).

Freezing of gait (FOG) is the sudden inability to take a step to initiate walking. It is often caused by different scenarios encountered during walking (triggers listed in the post). This can affect up to half of individuals with Parkinson’s Disease (PD) and lead to falls. Festination, or very short, shuffled steps/movements, often occurs before FOG and can eventually lead to very little movement (hypokinesia) or even no movement at all (akinesia). The first step in addressing festination and FOG is to become aware of the triggers.

In the spirit of the fall season, let’s talk about reactionary balance and its role in preventing falls. In simplest terms, reactionary balance is the ability to react quickly to sudden changes in your
environment. For example, when you walk your dog and your dog suddenly moves or changes directions in a way you don’t expect, you have to quickly step in a certain direction and adjust your body appropriately to prevent yourself from letting your dog pull you over. Our environments are dynamic and always changing, which is why it is important to train our ability to adapt to these changes! At BVC Physical Therapy, we can assess your reactionary balance and develop a customized balance training program that takes reactionary balance into account.
