Performs and oversees admissions (SOC), resumptions, re-certifications, and discharges by evaluating the patient in their home or community
Participates in On-Call rotation
Formulates/Oversees plans of care (POC)/Reviews all plans of care for accuracy, and appropriateness of documentation
Performs/oversees skilled nursing care in the patient’s home as necessary & provides teaching to all team members on skills and nursing care including but not limited to documentation
Evaluates patient’s needs and the effectiveness of care and makes changes and adjustments to POC as necessary
