*Provide us with a list of the specialists you see
*Please allow up to 10 business days for processing
*Make sure you do your part so we can do ours to ensure a smooth process

There will be a student immunization clinic during the Give a Kid a Chance event on Saturday, August 2nd.

Students MUST be pre-registered through the link below. We will not be able to take walk-ins.
