Dear Patients:
We are writing to inform you that, after careful consideration, Impact Health Direct Primary Care and Impact Health Aesthetics will be closing as of November 21st, 2025. Dr. Kara Sjogren, Dr. Michael Booth, and Nurse Practitioner Alice Bagby will no longer be seeing patients through Impact Health Direct Primary Care after November 21st, 2025. Our practice will be available until that time for your healthcare needs. It has been a great pleasure providing for your healthcare needs over the years, and it is not easy for us to give it up. We are truly grateful for the trust you have placed in us.
To ensure a smooth transition of your care, we recommend the following:
• Final appointments: Our last day seeing patients at Impact Health Direct Primary Care will be November 21st, 2025. If you have an appointment scheduled with us after that date, we will contact you within the next week to see if you would like to reschedule to a date prior to our closing. If you need to schedule a final appointment or follow-up before we close, please call us at your earliest opportunity. We will be seeing patients through November 21st, 2025.
• Continuity of care: We strongly encourage you to establish primary care with another physician or practice within this time frame/as soon as possible to continue your care. A list of local physicians can be found on the Ennis Regional Medical Center website, by calling the Ennis Regional Medical Center at 972-777-4290, or by contacting the Ellis County Medical Society at 972-875-9700. A list of direct primary care practices can be found on the DPC frontier website at https://mapper.dpcfrontier.com/
If you have trouble finding a new provider or practice please notify us as soon as possible and prior to our closing so that we may help you.
• Medical Records: Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your permission. If you would like for us to send your records to your new physician, please sign the enclosed authorization form and return it to our office as soon as possible. Alternatively, you may have the new doctor’s office fax us their own records request signed by you. They may fax the records request to us at 410-237-6747. Until then, your records will remain on file at our office.
• Medication Refills: If you are currently taking prescription medications and need a refill, please contact our office as soon as possible. We will continue to process appropriate refill requests until November 30th, 2025, but we strongly encourage you to establish care with a new provider before that time.
