We want to hear your voice! Complete the survey below to share your experiences of living with diabetes or experience of caring for someone who does. All participants ages 10 and up who live in the Eugene area will receive a $5 Amazon gift card. Your experiences will contribute to the creation of a program to improve diabetes health in the community in collaboration with DCCT volunteer and psychology graduate student, Chris Capron. We look forward to learning how we can better address your needs and thank you for your time and insights.
