The North Coast Resource Hub is an online directory that helps individuals and families find resources such as:

…and so much more.

This resource is incredibly valuable because it helps us quickly connect our community with services that fit each individuals unique needs.

This recognition highlights our team’s innovative work in Enhanced Care Management (ECM) supporting youth and families through strong referral pathways, meaningful partnerships, and true closed loop care coordination.

Through our Wellness Centers, we are committed to meeting students where they are and ensuring they have access to the care, support, and resources they need to thrive.

*MA Needed* The Humboldt IPA/Priority Care Center Is Hiring!

The Priority Care Center is currently seeking a dedicated and qualified Medical Assistant to join our team.
The Medical Assistant performs MA duties in a primary care setting, supports to the Priority Care Center (PCC) team and works with patients who have persistent and ongoing health care needs as appropriate.

The PCC team works closely with health care providers and other health care professionals to provide patients with support, guidance, education, and care coordination to help improve health outcomes.
