PentaHealth offers both back-to-school physicals and sports physicals to help assure that children as students or student athletes are healthy and ready for the new school year!

Parents, please bring any forms needed for school or sports to be completed by your child’s provider. If your child needs any vaccines, these can be given at the time of the physical exam.
