Great news! We have extended our business hours for Financial Assistance Walk-in Patients. We are now open Monday-Friday, from 8:30AM to 3:00PM for our Annandale and Fairfax Offices.

What does the renewal form look like?
A member’s renewal form should always come prepopulated with the information the state has on file about the member and his/her household. Its first page includes important information like the due date (found in a big bubble on the righthand side), and the member’s Case Number and Client ID.

¿Cómo es el formulario de renovación?
El formulario de renovación de un miembro siempre debe incluir la información que el estado tiene archivada sobre el miembro y su hogar. Update your contact information - Make sure Virginia Medicaid has your current mailing address, phone number, email, or other contact information.

Check your mail - Virginia Medicaid will mail you a letter about your Medicaid/ FAMIS coverage. This letter will also let you know if you need to complete a renewal form to see if you still qualify for Medicaid/ FAMIS.

Complete your renewal form (if you get one) - Fill out the form and return it to Virginia Medicaid right away to avoid a gap in you Medicaid/ FAMIS coverage.
