Locations: Northern Virginia.

We are looking for professional, skilled, compassionate, trustworthy, and kind caregivers with excellent communication skills to provide care to our clients.

Job Requirements:

· • At least 18 years of age and must able to read, write and speak English.
• All applicants must pass a criminal background check.
• Demonstrate compassion, responsibility, dependability and a cheerful attitude.
• Current First Aid & CPR.
• Proof of negative PPD test (chest X-ray if PPD was positive).
• Must be able to provide at least 2 professional references.
• Must possess effective communication skills and must have a reliable transportation.

Current Virginia HHA or PCA certificate (We can provide for a fee)

Job Types: Full-time, Part-time, Temporary, and on-call.

Pay: $11.00 - $13.00 per hour

POSITION SUMMARY:

The Registered nurse provides services to clients in accordance with the State Nurse Practice Act, agency policy, and accepted professional standards of practice. The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments. The Registered nurse participates in quality improvement activities within the Agency promoting overall compliance with State and Federal guidelines and professional standards of practice.

POSITION QUALIFICATIONS:

Current license as a Registered Nurse in Virginia.
CPR certification.
Strong communication and interpersonal skills.
Preferred home health care or hospital background.
Must have at least two years clinical experience.

PHYSICAL QUALIFICATIONS:

Must be able to stoop and bend; must be able to lift and transfer patients.
Must be able to travel to prospective patient’s place of residence.

DUTIES AND RESPONSIBILITIES:

Demonstrates leadership and provides direction to members of the patient care team through case management of patient home health episode of health care.
Performs comprehensive assessments of client status, including physical, psycho social, and environmental parameters.

Develops and/or follows an individualized Plan of Care that incorporates analysis of data and current scientific findings.

Provides skilled interventions aimed at achieving realistic client outcomes within a specified time period.

Reports changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes).

Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice:

Assessment skills as applied to the client, family/support system, and environment.

Promote the Agency and services to the public, participate in physician and community education related to services provided by the agency as registered

Provides in home supervision of Home Health Aide Services every 90 days or as needed for patient receiving aide services and documents the visit in the client record.

Performs other duties as assigned.

ALL RNs are required to wear a mask and record temperature during visits.

Flat rate $100 per Admission and $80 per reassessment and regular supervisory visits.
