Good Shepherd Hospice (GSH) is a non-profit organization that was created in 2001 with the merger of Good Samaritan, St. Charles and Mercy Hospices and represents a combined thirty years of experience serving the community. Our mission is to expand and enhance healthcare at the end-of-life for all individuals and their families living throughout Long Island, and to educate the community about hospice services. Good Shepherd Hospice helps patients and their families live each day fully, focusing on quality of life, control of symptoms and the things that matter to them the most. Catholic Health Home Care (CHHC) is a non-profit, non-denominational, home care agency certified by the Joint Commission. Catholic Home Care services more than 2,100 patients in their residences through the acute care agency and over 200 long term patients each day. The Home Care team consists of more than 550 dedicated professionals, para-professionals, and support staff to ensure that patients continue to have their home care needs met twenty-four hours a day, seven days a week. Specialty services include skilled nursing, physical therapy, occupational therapy, speech language pathology, nutritional counseling, medical social work, and home health aides. We have program specialties with highly trained staff to include pastoral care, behavioral health, infusion therapy, wound management, palliative care, and telehealth.
