We want to inform our valued patients that Advanced Family and Urgent Care Clinic, under the care of Melinda Williams, FNP, will be permanently closing as Melinda will be retiring!

📅 Important Dates
• Last Day for Patient Appointments: December 11, 2025
• Final Day of Operation: December 23, 2025

📁 Medical Records
Medical records will remain available through our clinic phone number until further notice.
If you need a copy of your records or would like them transferred to another provider, please contact us by December 23, 2025.

🩺 Continuity of Care
We encourage all patients to establish care with a new healthcare provider as soon as possible. Upon written request, we will gladly transfer your medical records.
