Hi New Hope Family!

We recently lost a provider but have hired a new one starting the end of October. Dr Fernandez will try to accommodate those that he can but please be patient with us during this transition. You may experience a delay in messages and prescription refills. We will be offering Telehealth Medicine a few days a week. Please call the office if you would like to be scheduled. We apologize in advance for any inconvenience this may cause.

TELEHEALTH

Effective Thursday, March 26, 2020 due to the COVID-19 virus we will be offering telehealth to patients. We do still strongly encourage you if possible to come in for your office visit to get a proper assessment but for those of you that cannot this is your alternative.

One of our providers will be available, you will have to wait in the virtual lobby. Wait times may at times exceed 30 minutes.

If you have an appointment at the office we encourage you to come in. If you cannot then please call the office to cancel your appointment. We are separating our sick from well patients.

***NO narcotics, controlled substances, nor antibiotics will be prescribed during this visit***

Dear Patients

As you are aware, the United States is experiencing a rapid increase in coronavirus disease 2019 (COVID-19) cases. It is our mission as a medical practice to keep you safe and healthy. To ensure your safety, we have instituted heightened cleaning measures at New Hope Family Practice. After each patient, all surfaces are wiped down with hospital grade disinfectant. This includes all chairs, countertops, hand rails, door k***s, pens etc.

We are asking all patients that have been exposed to COVID-19, either as a confirmed case or assumed case, stay home and self quarantine. If you have returned recently from hot spots such as South Korea, Iran, and EU nations please let us know and we will reschedule your appointment in 14 days.

If you have the following symptoms, please call to see if we have appointments available and self quarantine:

Elevated body temperature 99.9 F or higher
Body aches
Cough
Diarrhea
Runny nose
Sore throat
Shortness of breath

We are all in unchartered territory. With your help, we can limit the spread as much and as early as possible.
