MINOR SURGERY 9-22-25- Dr. Hal London

⚠️ Vewer discretion alert: Surgical images included

The procedure presented today is removal of a scalp cyst( pilar cyst). These are rewarding little procedures that are actually fun for me and yield great cosmetic results for my patients.

A pilar cyst, also known as a trichilemmal cyst, is a harmless, non-cancerous lump that develops on the scalp from the outer sheath of a hair follicle and is filled with keratin, the same protein that makes up hair and nails. These smooth, firm, mobile, and round cysts can be solitary or multiple, may run in families.

Inside of these cysts is a cheesy, foul smelling substance( think foot odor mixed with Parmesan cheese). They may become irritated and infected but usually are just unsightly with patients wanting them removed for cosmetic reasons.

Scalp surgeries usually present no problems because the blood supply is rich making them less prone to infection and in hair bearing areas the scar will be hidden. Even if there is no hair to hide the scar proper technique creates the smallest scar.

After numbing the area a single, small, superficial skin incision is made over the cyst deep to the top of the cyst but not into the cyst. This way the cyst can be dissected out intact to include the capsule that surrounds it(see photo). If the cyst ruptures during the procedure the foul smelling contents( consistency of cottage cheese) can be expressed then the capsule dissected and removed. If the capsule is not removed the cyst will reoccur.

Skin cysts, similar to the pilar cysts, can be found on the skin anywhere on the body and are removed in a similar fashion.

It’s that time of year again. When cooler weather sets in, it’s the time to get your skin resurfacing.

In fact, I just had my skin laser treated two days ago. No down time for me. I’m still seeing patients every day and I use the visible changes during my healing process to teach patients about skin resurfacing.

Most of my patients are Florida residents. Many have had significant sun exposure since childhood and their skin reflects it. Dark spots, premature wrinkling, and altered skin texture are just a few of the effects of sun exposure over years.

My carbon dioxide laser has many options for treatment settings. You can opt for a mild treatment with a couple of days of redness or an aggressive treatment with four or five days of recovery, and anything in between. Forehead lines are mostly from expression. You see them as horizontal lines across the forehead and vertical lines between the brows. Raising your brows and frowning are the motions that cause these lines. When you can see the lines without making the expression, they are etched in skin similar to scars and are permanent without treatment.

Why treat them? Facial lines project emotion to others. Frown lines give you the angry/stressed look. Forehead lines makeοΏΌ you look worried and aged. In the lower face marionette lines give you a sad/angry look.

Good news is that skin heals if you allow it. Skin cells are constantly sloughing invisbly and new surface cells replace them.

Toxins like Botox and Dysport weaken muscles of expression temporarily allowing the skin to heal. If you keep the muscle paralyzed by getting toxin every several months the etched lines can soften( slow process).

Most patients don’t want to wait a year for toxin alone to improve the etched lines and opt for filler in them as well. Filler will yield instant results and in this area the results are lasting.

Properly selected filler must be injected precisely to assure safety and great aesthetic results.

BEFORE AND AFTER FACIAL ENHANCEMENT- Dr. Hal London

This is mid and lower face filler. Softening folds in the mid and lower face will remove age from the facial appearance instantly.

Cheeks, nasolabial folds, lips, mouth corners, and marionette lines were all addressed in this enhancement.

Results last a year or more in many cases.

I am a physician and have been iniecting filler and toxin for about 14 years. There is no substitute for experience. For filler, the skill comes with knowing where, which skin layer, and how much of each FDA approved product to use. Too much filler, or injected in the wrong location and you look clown like, not enough and the results are suboptimal.

Please study the before and after photos presented today, especially the shape of the lips. Note: after the treatment, but before photos, I apply a small amount of petroleum jelly to best show where light reflects and shadows are cast.

οΏΌIn the after treatment photo you can see the distinct four points of light in the central upper and lower lips. This is achieved by adding volume to the five lip tubercles. There are three tubercles in the upper lip and two in the lower lip. The central tubercle of the upper lip should sit nicely between the two bottom lip tubercles as seen in the after treatment photo.

Another feature of this enhancement is how filler changes the surface contour of the lips, softening the the vertical lines. I like to finish my lip enhancements with support under the corners of the mouth. Can you see how the shadows at the corners of her mouth have been improved?

I love this procedure. It offers immediate results that last approximately a year.

This patient was brought in by her mother who told me that her daughter has been self-conscious about the bump on nose for many years. Teenagers are so critical of each other, sometimes bordering on cruelty. Having the opportunity to enhance someone’s self image is a prime motivator in my practice and I was happy to help. This is an off label usage of filler, but with parental consent I felt comfortable helping this young lady.

οΏΌIt’s literally a 10 minute procedure with lasting results. If she likes the result and wants to move forward with a major οΏΌsurgery to correct the problem permanently, this will be an option for her. Otherwise, she can repeat this procedure safely about once a year or as needed.

Adding facial filler in the proper amount in the correct location and depth is key. In patients with full facial features I try to create a more heart shaped face by adding filler at the zygoma, as this should be the widest part of her face.

The photos presented today are of my patient who had anοΏΌ ear piercing that had completely torn through. After anesthetizing the earlobe, I freshened the wound edges and re-approximated the skin with sutures. I allowed the wound to heal for a week then removed the stitches. I re-pierced the ear after six weeks. οΏΌA small scar will remain, but this patient can once again wear her favorite earrings.

Dr. London specializes in anti-aging and beautifying techniques, such as Botox, wrinkle and line fillers, lasers and many other of the latest aesthetic treatments. He studied at the Philadelphia College of Osteopathic Medicine and qualified as a medical doctor in 1987. He practiced as an emergency physician for 28 years before pursuing his passion of minimally invasive cosmetic medicine and non-surgical facial treatments. His medical spa facility, London Medical Spa, which he established in 2012, is located in an 1880’s restored Victorian residence in historic downtown Fernandina Beach. Even with a thriving practice Dr. London also finds time to perform as the drummer for the area’s top rock band, the Amelia Island-based Honey Badgers.
