EARLOBE REPAIR- Dr. Hal London

The photos presented today are of my patient who had an ear piercing that had completely torn through. After anesthetizing the earlobe, I freshened the wound edges and re-approximated the skin with sutures. I allowed the wound to heal for a week then removed the stitches. I re-pierced the ear after six weeks. A small scar will remain, but this patient can once again wear her favorite earrings.

SKIN GROWTH REMOVED. WHERE IS THE SCAR? - Dr. Hal London

Will surgery using a blade will always leave a scar? YES.

The question should be: “How will the scar look after the surgery?”

These photos are of my patient before and after her surgery to remove a skin growth. It was visible on her neck for many years. She didn’t like the way it looked and would hide it with her hair.

It was a seborrheic keratosis, a very common finding in older adults. They are genetic and have a characteristic appearance. I see patients frequently for removal of this non cancerous growth.

Insurance won’t pay for removal and testing so my patients come to me rather than their dermatologist knowing that my priority( after knowing it’s not a skin cancer) is a great cosmetic result.

RESHAPING LIPS WITH FILLER - Dr. Hal London

The photos presented today are of my patient before and immediately after her enhancement.
Especially notice her upper lip before treatment.
It’s an “M” shaped lip with the center projected more down and forward than the sides. Also notice her left upper lip is more arched than the right side. Her Cupid’s bow( central upper lip) is asymmetric and indistinct.

The after treatment photo was taken by me 20 minutes later after addressing these concerns. The swelling will reduce by about 10-20% over then next 48 hours leaving her with a lovely natural looking enhancement.

My techniques:

Facial aesthetics is very artful. It not only involves the manual dexterity of physically using the needle but an eye for what looks best and the knowledge of what is needed to create these changes.

The most artful procedure I perform is lip enhancement. Every pair of lips are unique and present new challenges. That’s good for me. Keeps it interesting. What’s good for my patients is that most times all they need is a little reflation of volume and the lips resume their more youthful appearance. It’s not always that easy.

The real challenge is when their lips have been filled by someone else before I’ve seen them. Too often they are unnaturally shaped, not proportional, and uneven. The choice is to dissolve the filler with hyaluronidase enzyme and refill them in a couple weeks or try to “fix”
them by adding volume where needed.

⚠️ It’s better to get them done well the first time by a trusted doctor.

MIDFACE AND UNDER EYES -Dr. Hal London

In the best candidates, the combination of  filler under the eyes and in the mid face is one of the best fixes for the aging face. It’s an easy filler treatment, but must be done precisely to avoid complications. It takes about 30 min after 20 minutes of numbing cream. Some mild swelling and occasional bruising possible afterwards. 

The under eye is a very unforgiving area to add filler because there is no soft tissue to speak of. It’s just bone,muscle, and skin in most people.

Blending the mid face with the under eye area takes finesse, an aesthetic eye, and keen knowledge of anatomy. There is no substitute for experience here. 

Results last a year or more in many cases.

YOU WILL NEVER LOOK AT SOMEONE’S BROW THE SAME WAY AGAIN- Dr. Hal London

I’ve been injecting neurotoxin for wrinkles for about 14 years. Some common brand names of FDA approved neurotoxins are Botox, Xeomin, and Dysport.

How does it work?

Neurotoxin is a muscle weakener used to slow or stop movement of the muscle injected. It’s a temporary action that slowly goes back to normal function over three months. 

Toxin in the forehead

Across the forehead is a thin sheet of muscle called the frontalis. During expression the frontalis contracts, raising the brows and creasing the skin of the forehead( dynamic wrinkles).  Over years this horizontal skin creasing becomes permanent and can be seen even without expression( static wrinkles). The more expressive you are the deeper the lines become.

Botox to the rescue! 

Neurotoxin injected in this muscle will weaken the frontalis. This limits the amount of skin creasing and prevents or softens horizontal forehead wrinkles.

This treatment comes at a cost, both financially and anatomically. In most practices the patient cost to treat this area is between $100-$300. The other, arguably more important, cost is anatomic.

When the frontalis muscle( the only brow elevator) is weakened, the eyebrows can drop and worsen or cause upper eyelid hooding. This is an expected side effect and limits the use in patients that already have flat brows or eyelid hooding. It will make it worse and it is not reversible. Creates unhappy patients.

Observe others, lots of examples on social media. See that shiny forehead, low brow and upper eye lid hooding? Too much Botox!

What to do?

I select my patients properly . In many patients over the age of 50 the brows tend to drop/ flatten and upper lid hooding develops. Adding toxin to the forehead will exacerbate this and it’s not reversible. The over-injected patients are stuck with this undesirable look for several months. My practice has changed over a decade and now I limit the use of toxin in the forehead in patients over age 50. I either avoid forehead treatment totally or give microdoses, precisely placed, to limit the brow drop.

Paul Alexander, who passed away in 2024 at the age of 78, was one of the last people in the world to live inside an iron lung—a towering, tube-like machine that kept him alive for over 70 years. At just six years old, Paul contracted polio during the devastating 1952 outbreak in Texas. The virus paralyzed him from the neck down and left him unable to breathe on his own. Doctors placed him in an iron lung, telling his parents he might not survive the night. But Paul defied the odds, beginning a lifelong journey of survival within the humming, steel confines of the machine that would become both his prison and protector.

Despite his physical limitations, Paul lived a life of astonishing courage and determination. Using a plastic stick held in his mouth, he taught himself to write, eventually earning a law degree and becoming a licensed attorney. He also authored a memoir, *Three Minutes for a Dog*, typing each word painstakingly with his mouth. Paul never allowed his condition to define his worth. Instead, he became a symbol of resilience, dignity, and the power of the human spirit. Friends and followers from around the world drew inspiration from his story and admired his sharp mind, wit, and unwavering strength.

Paul’s passing marks not just the end of a life, but the closing of a chapter in medical and human history. His decades spent inside an iron lung are a sobering reminder of the devastating reach of polio, and the miracle of modern vaccines. But beyond the machine and the diagnosis, Paul Alexander leaves behind a legacy of perseverance and purpose. His life—brave, difficult, and deeply meaningful—touched thousands, reminding us all that even within the harshest limits, hope, intellect, and humanity can flourish.
