The Ergotrid is the area between the upper lip and the nose. It elongates and flattens with age. It’s a little discussed target for enhancement that I treat regularly.
The non-surgical approach involves everting the upper lip to give the appearance of a shorter Ergotrid. This must be done in a reserved fashion and with artistry to yield a natural looking result.

Her bottom lip had normal age related flattening and inversion. Adding just the right amount to the central 1/3 did the trick. Lovely.

EXPRESSION LINES and LIPS - Dr. Hal London.

Expression lines are very common. Everyone moves their face during expression. As kids, the skin folds during expression and springs back with no lasting lines (dynamic wrinkles). With age, the skin folds and then leaves a line or wrinkle even when not expressing (static wrinkles). Static lines form faster in sun-exposed skin and in smokers. I teach people how to use their faces to minimize expression, but habits are hard to break.

The good news is that blended filler and Dysport, expertly placed, can really help with etched expression lines.

NON-SURGICAL NOSE ENHANCEMENT 4-30-26 Dr. Hal London

She had a bump on her nose forever but It literally took 5 minutes to fix it.

Features that make us all unique and special are sometimes targeted as defects in those who have them. Maybe it’s a mole or a pimple or thinner lips, whatever it is there will be someone who doesn’t like the way they look because of it. Today I fix the nose bump.

Noses come in many shapes and sizes. They are front and center in the face and even small β€œirregularities” can make a person self conscious and insecure. One of the most common complaints is a bump on the nose bridge(dorsal hump). This is often genetic but can be related to prior trauma and other causes. The good news is a dorsal hump is one of the easiest cases to address on the nose.

Tiny amounts of filler placed strategically can mask the bump and create a near perfectly straight nose on profile. It’s one of my favorite treatments to perform because it’s instant and so rewarding.

I’ve had more than a few patients pick up the hand mirror and cry after my nose treatment. It’s true, they had struggled with the appearance of their nose all their life, avoided the side profile in conversations and photos and suffered low self esteem based only on this small feature. In 5 minutes I change all of that and it’s a bit overwhelming for them. It’s a happy moment for the patient and the doctor too. That’s what it’s all about.

Results last about a year.

NEUROMODULATOR OFF LABEL INDICATION- Dr. Hal London

Perioral lines are a problem commonly seen in my practice, like everyday. Mostly effecting woman, these lines add age to the face. Men have whiskers and are thought to add protective support to the skin.

Women hate them, really hate them. I’m asked what can be done to soften or remove them. First, why do they develop?

Under the skin around the mouth there is a circular muscle( obicularis oris) , like a ring around and under the lips. When this contracts the mouth purses. This action allows you to speak and contain food and drink in your mouth, kind of important yes?
Also kissing, whistling, and other less critical motions are produced when o. oris is working. These motions fold the overlying skin and overtime cause multiple vertical lines resembling a barcode especially above the lip. Horrible.οΏΌ

What can be done to help? The solution is multi factorial. First, stop or limit the motion.
I try to teach my patients how to use their face so as not to produce or worsen existing expression lines. This is a tough sell because
expressions are habits and habits are hard to break. That’s where using judicious amounts of neuromodulators like Dysport or Botox come into play.

These medications weaken muscle temporarily, and when used properly, can soften these lines and prevent worsening.
The skill comes in knowing where to inject and how much. Too much medication and you are drooling your soup. Not enough and the effect is insufficient. I have seen many patients who have gone to someone who was not experienced in this treatment and suffered three months of disability. Horrible twice.

In addition to using neuromodulator, I employ my CO2 aggressively to soften these lines. Multiple treatments might be needed depending on the depth of the lines. I will often use blended filler in some of these lines to great effect as well.

DYSPORT TREATMENT- Dr. Hal London

I’ve been using Dysport exclusively for 15 years on my patients. It’s a neuromodulator like the well known Botox. Botox and other neuromodulators are good choices is you want to spend more money but get the same or less effective result. I am not supported by any drug company.

It works by inhibiting the effect of neurotransmitter at the neuromuscular junction of any muscle in the body. It’s used in many areas off the face, in the GI tract, in the urinary system, and even for migraines.

Dysport works a bit faster than Botox. I’ve noticed it working on my own face within 6 hours compared with other neuromodulators that start their effects in a few days. Dysport spreads more than Botox so the injector must take that into consideration( difference btw a dime and a quarter).

In all my years of using it and after many thousands of injections, I have NEVER seen any unexpected side effects. I use it on myself, my staff, and even used it to treat my beloved Mom when she was alive.

I treat as many patients with Dysport as I do filler. It’s a large part of my busy practice.

That said, it does take skill to perform the injections properly and get great results without untoward effects. Proper dosing and injection site location is critical.

If you want to hate your treatment go to a new injector and watch your eyebrows fall and flatten while your eyelids get heavy from an overly aggressive forehead treatment.
