MARIONETTE LINES- Dr. Hal London

Everyone hates them. I love them. Why?
Because they keep me busy.

Marionette lines or folds are common….very common. I treat them every day. The good news is that it’s an easy filler fix providing instant and dramatic results. It’s typically a pick up the mirror and say WOW! moment for my patients and therefore very rewarding for me.

It’s a safe filler treatment when performed with precision and keen knowledge of facial anatomy. This area tends towards bruising but it’s usually minor and easily covered with makeup.

I have some tricks developed over the years, especially in lifting the mouth corners. This removes the sad/angry look.

The treatment is affordable and I charge for only one area of filler. Results last about a year.

FACIAL ENHANCEMENT 2-24-26 - Dr. Hal London

I saw this lovely lady as a new patient this week. Short story is she had been following my work for a couple of years on Facebook and felt she needed to do something to look a little younger. In thirty minutes, using filler and toxin, I was able to restore her youthful contours and soften etched lines. She left a kind review the next day.

The details:

Knowing how the face ages is critical for creating a natural looking enhancement. We all age similarly but also differently depending on many factors too broad to explain for a post this brief.

As I recall, her main concern was her lower face, around her mouth and chin. In consult, I explained to her how the face contour changes as we age and how we lose volume in our faces even if we don’t lose weight on the scale. We even lose bone! I commonly see asymmetry due to bone resorption in patients over the age of 50. In fact, she too had lost more bone on her left face and required about 30% more filler there to create symmetry.

Her lips had also lost volume and shape. Adding a small amount of filler in the correct places gave her more youthful looking lips without looking over filled and wierd.

CHEMICAL PEELS- Dr. Hal London

One of my favorite treatments. Why? Because they work! Chemical peels are a simple, effective way to rejuvenate your skin. They help with fine lines, dark spots, skin texture, and glow.

The actual procedure is very simple and takes 20 minutes.  It does involve 3-4 days of downtime when you won’t be looking so great. Truthfully, I do peels on my own skin four times a year and continue seeing patients.

On days one and two, the skin is tight, shiny, and slightly tan looking. In the majority of patients, skin starts peeling on days three and continues for 1 to 2 days. The new skin that is revealed will be slightly pink and fresh like a blister with the skin removed.

The only discomfort for the whole procedure is a little stinging lasting 1 to 2 minutes while the chemical is applied. My Mom always told me “that’s how you know it
it’s working” after applying that red stuff( merthiolate) when I was a kid. Kids believe anything.

The photo presented today is on day four of her chemical peel. The skin is tight, shiny and bronze color. If you look carefully, you can see that her skin has started to peel.

It takes very little filler to make a difference. My opinion, one syringe of filler is likely too much.

I never measure my filler amounts. I inject until I’m done and my patient looks fab.

In practice, it’s better to under treat than over treat. Adding a drop more filler at two weeks is easier than dissolving it. Patients appreciate the subtle changes too.

BEAUTIFUL LIP SHAPE - Dr. Hal London

The upper and lower lips are very different.

Upper lip: The central upper lip has a distinctive shape. The Cupid’s bow can be visualized by imagining a bow and arrow with the tip pointing downward (central tubercle)
The two philtral columns are the bow strings.

The bottom lip: fuller centrally( width of the nose) with a subtle cleft in the midline( in some people not others ). The central tubercle of the upper lip sits in the cleft of the lower lip.

Both upper and lower lips taper gradually towards the sides. Perfect symmetry is the goal.

Pro tip: A drop of Dysport in the chin keeps the lip shape longer.

FACIAL REJUVENATION 2-12-26 Dr. Hal London

It’s Friday, how about a lesson on facial shape, lines around the mouth, and lip contour? 

It’s another example of how I strategically place dermal fillers to enhance the face.

With before and after photos you can often see the improvement but not know what has changed. Please allow me to show you. First, knowledge of how the face ages.

Please take a moment to study the before and after photos. Notice the “shape” of her face before treatment. Do you see that before treatment, her face has a more “square” shape, and after treatment, her face appears more heart-shaped? That’s filler in the mid-face.

The “heart-shaped face” is desirable and signifies youth. Why? Faces change shape with age. They sag from volume loss( bone and fat) and skin laxity, subtly changing facial shape over the years. This can be demonstrated by what I call the triangle theory.

Imagine inverting a triangle so that the base( widest part) is now at the bottom instead of at the cheekbones. You can see this demonstrated in the photo presented today.
Adding filler in the mid-face restores the triangle so that the base is again at the top. Nice.


The mouth

It’s an area that ages faster than other areas of the face. Movement is the culprit here, and most of it is voluntary. Talking, eating, smoking, drinking from straws, and expressions all crease skin and cause lines over time. “Smoker’s lines” can be seen in non-smokers too. Some of these movements can be controlled voluntarily through behavior modification , and I help my patients work through that.

Combine excessive movement with volume loss, and you see the perioral area age prematurely. Add sun damage, and the problem multiplies.

Lips

Lips deflate like a tire when they lose fat. What you probably did not know is that you can control how your lips age.

The lower lip- It is especially vulnerable to aging because of facial expression. What I see clinically is that the bottom lip in many patients over age 50 is "smushed" by the pressure of the chin and other muscles. These muscles press the lips together and flatten them. It’s a habitual, voluntary motion that can be stopped with great effort. Habits are hard to break, but just being aware is the first step. Stop"smushing" please.

Please examine the photo before treatment and notice how the volume of her lower lip appears now as a bulge seen just below the red border of the lip( arrows). Enhancing the lower lip with filler masks this feature providing a more youthful contour. You can see what a small evolving bruise looks like on her left lower lip; it's no big deal.

The upper lip- It loses shape more than volume, and restoring shape, especially the Cupid's bow of the central upper lip, is a beautiful addition to any lip enhancement.

Injecting fillers involves considerable technical skill and artistry. There’s a lot of thought that must go into the process before the needle ever touches the skin. Who you go to really does matter.

Subtle improvements can be seen in these before and 2 week follow up photos. This natural looking improvement is best seen by examining the before and after photos in sections. Start at the forehead and work downwards. Look at the upper, mid, and lower face separately.

Notice the smoothing of the forehead and relaxed brow but without the dreaded eyebrow drop that can happen with too aggressive toxin.

See the gentle lift in the midface?

Look at her mouth corners. Doesn’t she look a bit less sad with her corners lifted and marionettes filled?

Etched lines present quite the challenge. Etched lines are mainly from expression and most frequently found in patients with extensive sun exposure. In smokers, you see these deep, permanent lines predominantly around the mouth.

Superficially injected filler can soften these lines. Using carefully placed Botox or Dysport plus behavioral modification can add additional benefit.

These lines are not going away and worsen with age but I can help.
