Insurance companies create a monopoly for patients and providers. This typically occurs at the demise of the patient both in terms of health but also financially. Over the years I have seen insurance payers find every way possible not to allow benefits that the individual pays for in premiums, and deny payments to the providers for quality and skilled services they provided. The article attached explains the process well.
More commonly than not as in-network providers, we are driven to provide care that the insurance company tells us we can do, which often is not what the patient needs. We have all heard doctors take an oath to the extent "do no harm", and while providing care based on insurance limitations is not directly harmful, omitting services a patient needs to ensure payment and to follow contractual obligations certainly is.
Many people may not know that hospitals, clinics, and doctors offices would love to offer cash plans for patients who wish to receive all-inclusive care for everything that they need to get healthy without limitations determined by insurance companies and their algorithms for determining benefit approval.
