Impalement 🗡️ Injury

Shown is a 32-year-old male passenger involved in a high-speed, roll-over car accident. He was not wearing a seatbelt. The car struck a chain-link fence, and the patient was impaled with a fence post to his right thorax.

Windshield ➡️ patients right chest ➡️ exit back of passenger seat

Presentation ⬇️
1. Complex Type 2 Impalement
2. AAO X3
3. GCS 15
4. No active hemorrhage (hemodynamically stable)
5. Chest X-ray = no identifiable acute pulmonary injury
6. OR ➡️ intubated, wound exploration, bleeding control, object removal, devitalized soft tissue resection, wound washout, reapproximation of muscles, surgical site closure with staples, placement of right-sided 28 French chest tube.
7. POST-OP ➡️ ICU

Impalement Management ⬇️
1. Careful extrication by EMS. Minimal manipulation of object.
2. Rapid transport to trauma center
3. DO NOT remove object on scene. Leave in place.
4. Imaging if needed
5. OR
6. Vascular control BEFORE object removal.
7. Repair of subclavian artery/vein are often necessary
8. Resect necrotic tissue. Preserve lung tissue to prevent empyema.
9. Contaminated wound = lavage, debridement, antibiotics, and tetanus prophylaxis 💉
10. Close the chest when situation permits

The patient survived the injury and was discharged home 🙏

Brownsburg Officer got a search warrant and arrest warrant for:

Justin Patrowski, 34 years old

Search warrant served with no incidents.

He is facing:

Intimidation Level 6 Felony

Felon in Possession of A firearm: Level 4 Felony

Possession of a controlled substance

Dealing

NEW: man taken into custody with the help of Brownsburg SWAT.
Working to get confirmation on who was taken into custody and why.

Witness says home in the 500 Block of Alpha Avenue near Williams Park was watched “all day.” Brownsburg PD tells me, “wanted subject in custody.”

No one injured.
