Good time out in the French mountains the last couple weeks. I feel very lucky to have been able to spend time with my wife and friends, support athletes I work with and meet new ones, crew .sports.dietitian during CCC and see him get things done, and of course be in a place where the mountains are a bit incomprehensible.

Also happy to be home now after 2 days of food poisoning and travel (a bit of a forced taper), seeing my pup and settling back into a groove (well for a few days before leaving for 😂)

This MRI is from a runner with lateral knee pain (only during normal life, never during running). The got an MRI to determine the structural cause of the knee pain that’s been around the last 5 years (thought to be chondromalacia).

As you can see, the MRI shows no lateral tissue damage, no articular damage (where chondromalacia would show up, and no swelling. Where this person has pain is completely normal.

What it does show is mild bone edema in the medial femur. This sounds scary but this is actually a pretty normal and common finding among runners. It also shows some tedinosis in the patellar tendon which is also a pretty normal finding. This runner happens to have absolutely no medial pain or patellar tendon pain.

Based on the fact this runners knee is BETTER with running and there were no findings that related to their pain means it’s likely something muscular (strength or stability) causing the issue and when they are running, things are activating better and a bit more smoothly.

This happens all the time
 Either an MRI does not confirm the patient symptoms OR the MRI shows issues where the patient has no symptoms at all. Beware of what you read!

If anyone ever has MRI findings that they want more help deciphering, I’d be happy to help.

Hear it over and over again
 athletes stuck in the injury cycle.

pain -> rest -> no pain -> train -> repeat

It’s because rest drives down irritation but it doesn’t solve any problems.

If you want to get better, you have to change something to break the cycle.

This year, I have the incredible opportunity to compete at the World Mountain Running Champs for the Short Course 45km in Spain this September. I also have some shorter races and other mountain objectives that I’m looking forward to this year. I really enjoy both ultra and sub ultra distances, so as long as I am on a trail, I’m happy! Strength training has been my missing component for years and is truly the thing that will help me train and race more consistently. “I have been a pro trail and mountain running athlete with Vibram for three years and I raced my first 100km this spring at Gorge Waterfalls. I’m most looking forward to learning how to take care of myself so that I can continue to run and race these long distances for many years to come. My goals are to both increase speed and strength for performance in the short term (races are getting faster and faster!) as well as maximizing long term durability. “Hello, Ya’at’eeh, Loloma, Nu’ Hosava yan Hopimatsewa (My Hopi name is Hosava/Hopi). ShĂ­ éí Hosava yinishyĂ© (My name is Hosava/DinĂ©). I grew up in a small village called Hotevilla on the Hopi reservation and I am an enrolled member of the Navajo Nation. I now reside in Flagstaff, AZ under the San Francisco Peaks (Nuva’tukya’ovi/ DookÊŒoÊŒoosƂííd). Running has been a big part of my life and I continue to run to honor the Hopi and Navajo Nation. Through my running experience, I have been given many opportunities to meet other fellow runners, inspire native youth, and travel across the country. Along the way, I have received support as a sponsored athlete from Run Flagstaff and Hammer Nutrition in pursuit of qualifying for the Olympic Trials in the Marathon. I am looking forward to running healthy, staying consistent, and building upon my physique to be a stronger runner. I am preparing to run in a professional race called the Marathon Project in December of this year. I will need all the help I can get which is why I am grateful to be working with Dr. Kameron Harder to assist in my preparation going into this race. In the past, I have not had a formal strength coach and I am really looking forward to working on my musculoskeletal system to carry my body over 26.2 miles.”
