Why do we need a health history form completed every 6 months?

A health history form is crucial because it provides a comprehensive overview of a patient's past and present health, enabling healthcare providers to make informed decisions about diagnosis, treatment, and preventative care. It helps identify potential risks for cleanings and helps us tailor care plans to individuals with special needs. It also provides us with important information about your child's allergies, and medications. We normally request these forms to be completed 24 hours before your child's visit to ensure there aren't any factors that will interfere with an appointment. For example, recent surgeries, concussions, infections, artificial joints, certain heart conditions and recent extractions of wisdom teeth may prevent a child from getting a cleaning.

Completing these forms in advance helps us keep your child and their smile happy and healthy!
