Meet Nicole, one of our incredible RN Case Managers in Michigan! With 10 months at Swan Hospice, Nicole brings compassion, collaboration, and heart to every patient and family she serves.

Nicole works together with families and patients to meet them exactly where they are in their journey—educating, supporting, and involving them in a plan of care that honors dignity, respect, and comfort.

To Nicole, Swan Hospice feels like family. She describes our team-based approach as “having never-ending support in your pocket,” allowing her to truly practice the art of nursing in the most meaningful way.

Ashleigh is one of our incredible Home Health Aides, providing direct, compassionate support to patients and families. Since joining Swan Hospice on July 31, 2023, she has played a vital role in comforting patients at the end of life—assisting with bathing, dressing, grooming, and offering steady companionship and emotional support. 💜

Ashleigh shares,
