His diagnosis include: Chronic Lung Disease, Pulmonary Hypertension, Developmental delay, gastrostomy, and tracheostomy. Duties to include: trach care/ suctioning, G-tube feeds, oral feeds, medication administration, PVT/nebulizer treatments PRN, pulse ox, and diaper changes. He is ambulatory but requires help with ADLs.
