Lakeland Care coordinates care for eligible elders and adults with disabilities in 31 counties. Lakeland Care, Inc.

(LCI) is a Wisconsin-based managed care organization (MCO) providing long-term care services and supports to eligible frail elders and individuals with physical and intellectual/developmental disabilities through Wisconsin’s Family Care program. Lakeland Care’s goal is to support individuals to live as independently as possible in their homes and communities by providing high quality, cost effective services and supports using a member-centered, outcome-based approach. Falls Prevention Awareness Day is a time to highlight the importance of reducing fall risks and supporting safe, independent living for older adults. Simple steps like removing tripping hazards at home, wearing supportive footwear, using mobility aids when needed and staying active with balance and strength exercises can make a big difference.

Since September is also Healthy Aging Month, it’s the perfect reminder that staying active, connected, and mindful of safety are key parts of healthy aging.

Lakeland Care was proud to be a part of the Fond du Lac County Care Giving Conference yesterday!

LCI staff, Julie Reindl and Andrea Prendota represented us at the LCI booth, and Shelly Pulda, a long-standing member of the Dementia Friendly Fond du Lac County Conference Planning Committee welcomed participants and helped the day run smoothly. It was a wonderful day of community education and support for caregivers, capped off with an inspiring keynote from Sharon Lukert.

We’re grateful for the opportunity to connect, share resources, and continue supporting those who make a difference in the lives of others.
