Have your authorization form printed out as your form of payment or there’s a $5 charge back fee for printing.

DOT Physical walk-ins r from 9am - 11:30am & 1:00pm - 6pm. Friday only at our Forest location located at 321 S. Main St.

Our schedule is already tight, so get n where u fit n! DO NOT CALL! Phones will not b answered but u can leave a message to schedule an appointment. All voicemails r returned within 24 hours (601-564-7141).

DOT Urine Collector Course

We will be conducting DOT Urine Collector courses starting in February. Let us know if you or someone you know are required to have this course.

The certification comes with a 5 year expiration date. All materials are provided and mock collections will be conducted.
