Nav Central Nav Central, LLC, is a Comprehensive Clinical Command Center in Forney, Texas.

Nav Central is a patient centric service providing telephonic triage, telemedicine, and interventional care but most importantly, Healthcare Pathway Management, to patients across the United States. Nav Central is currently managing over 6,000 patients in 3 states with contracts in place to expand our population management to over 20,000 lives by the end of 2016. Nav Central’s model offers a bundling solution to provider groups, including risk sharing, program evaluation, development and management, as well as a sophisticated technology platform to support at risk provider groups in risk-based contract agreements. CMS Bundled Payments for Care Improvement (BPCI) Initiative represent an important advance in the organization and financing of health care services. They are a key component in the market transformation to value-based reimbursement and include economic incentives for care coordination among health care providers leading to lower costs, higher quality of care, and increased patient satisfaction. Nav Central is an integral solution to this philosophy and currently provides care to over 5,000 BPCI patients across 3 states with a proven ability to reduce the return to admission (RTA) rates and demonstrated improvement in patient outcome and satisfaction. Our hearts and prayers are with the Terrell Police Department this morning after a tragic night. We stand beside you for anything you need. 🙏🏻🙏🏻🙏🏻

On Sunday, December 8, 2024 around 11:00 pm, Officer Jacob Candanoza, Badge Number 6190, initiated a traffic stop in the 1600 block of S. State Highway 34.

Officer Candanoza requested a cover unit and units were dispatched. Within moments of this dispatch, the Terrell Police Department Communications Division received two 9-1-1 calls regarding an officer being shot. They then received a radio transmission from a citizen giving the same information. Responding officers were advised of this information, along with Terrell Fire Department and CareFlite being dispatched.

Officers arrived on scene and confirmed an officer had been shot. Medical attention was administered immediately and Officer Candanoza was transported to Baylor Scott and White in Forney. Unfortunately, Officer Candanoza did not survive the injuries sustained.

Officer Candanoza has been a police officer with the Terrell Police Department since July 2024. Our deepest condolences go out to his family as they navigate this tragic time.
