DR. DELUCCA WILL BE RETIRING AND CLOSING HIS PRACTICE ON JUNE 30, 2023.
Please call our office at 515-576-5000 before 6/30/23 to have your records transferred free of charge to the practitioner of your choice. We will gladly provide you with a listing of all the Board-Certified Ears, Nose and Throat Specialists within a 100-mile radius.
AFTER JUNE 30th all patient records will be sent to Desert River Solutions to be the custodian of medical records. STARTING ON JULY 1st please start requesting your medical records from Desert River Solutions. They will charge a fee of $25 for records transfers. To request your medical records after 6/30/23 please visit the website below to sign and return the medical records release form. All instructions are on that webpage:
