• Sacroiliac Injection – Inflammation of the joints on both sides of the spine where it joins the pelvis can cause low back pain. Steroid and local anesthetic are injected to diagnose and alleviate the pain.

was recently promoted to Associate Professor of Neurology at the University of Colorado (Denver).

To achieve this status, Dr. Wilhour completed many hours of dedication including research, teaching, seeing patients, serving as Director of Residents & Fellows and as a life coach for female medical students and residents as well as authoring various articles/findings. She is also a busy mom to a son and daughter.

INSPAN® represents a significant advancement in spinal care that combines nerve decompression with stabilization for long-term relief of back and leg pain.

Symptoms that may indicate a COMPRESSION FRACTURE …

• Sudden onset of severe back pain
• Severe pain with coughing or sneezing
• Increased pain with deep breathing
• Worsening pain when standing or walking
• Pain when changing position or bending
• Some pain relief when lying on one's back

Spinal fractures can occur anywhere in the spine. They are most common in the mid-back (thoracic spine) or upper lumbar segments, such as L1.

Spinal compression fractures are often the result of osteoporosis and most commonly occur after a fall in females over age 70.

At Advanced Pain Management & Spine Specialists, we perform minimally-invasive procedures to repair compression fractures, including:

VERTEBROPLASTY: A low viscosity cement that is injected directly into the collapsed vertebrae to stabilize the fracture and relieve associated back pain.

KYPHOPLASTY: A cavity is created and filled with cement to stabilize the bone and restore some or all vertebral height.

These procedures are safely performed while the patient is comfortably sedated. Relief is often experienced immediately or within 1-2 days.

Dr. Daitch's "GOOD TO KNOW"

“Sciatica” refers to pain that runs along the sciatic nerve in the lower back and extends down one or both legs. It is the result of a herniated disk or bone spur in the spine that is pressing on the nerve.

Some patients describe the pain as shooting, or sharp jolts. Others describe it as burning, tingling or stabbing.

In our Lee County pain management office, we offer several options to relieve sciatic pain. These include:

• InSpan – placement of a minimally invasive interspinous fusion device to create room in the spinal canal to take pressure off pinched nerve roots, relieve spinal stenosis, and stabilize the spine.

• Epidural Steroid Injections – Local anesthetic and steroid are injected under fluoroscopic guidance to treat persistent sciatica and nerve root pain.

• Selective Nerve Root Injections – Local anesthetic and steroid are injected under fluoroscopic guidance along the exiting spinal nerve root to treat sciatic pain emanating from the nerve root.

• Percutaneous Disc (Perc D) Decompression – By removing a small amount of disc material, pressure on the nerve root is alleviated through this minimally invasive procedure. This is beneficial for patients who have not experienced adequate relief from epidural or nerve root injections but do not need open spinal surgery.

SW FL interventional pain management physician Jonathan S. Daitch, MD discusses the positive results of the MILD (minimally invasive lumbar decompression) and MinuteMan procedures with a recovering patient at her post-op appointment.

Evelyn had previously experienced difficulty walking or standing, enduring miserable pain that ran from her lower back down her leg. In this interview, Dr. Daitch reviews how the removal of a small amount of offending ligament and implantation of a stabilizing device - the combination of two minimally invasive procedures performed through a 1-inch incision - have given the patient restored comfort and ease of motion.

Now excited about resuming a life where day to day tasks are performed with ease, Evelyn is especially pleased to know she can cook again and stand for long periods without experiencing pain.

MILD (minimally-invasive lumbar decompression) is an outpatient procedure that relieves pressure in the spinal canal through an incision the size of a baby aspirin. This outpatient procedure is quick (about 30 mins) and covered by Medicare nationwide.

Results are typically experienced within a few days, providing:
• Reduction in pain
• Restored mobility
• Increased standing time of up to an hour
• Increased walking distance

Minuteman is performed through a one-inch incision and does not involve cutting any muscles or bone. There is minimal blood loss and you return home the same day - generally within an hour after the procedure ends. There is also minimal discomfort after this type of stabilization procedure.

In this, a heavy-duty toggle bolt (with wings) is placed through the rear elements of the spine. Once the bolt is placed, the toggle wings are opened. The bolt opens the spinal canal and creates room where the pinched spinal nerve roots exit. The wings catch on the spinous processes and a “nut” tightens everything into position, stabilizing the spine. Bone cement is added to further ensure proper fusion/stabilization of the spine.

The device is a permanent implant and can be surgically removed in the future if needed but does not burn any bridges should further treatment be necessary.

Minuteman can be especially beneficial for patients who have been offered more extensive surgical fusion procedures and prefer a less invasive option or for patients who may be unable to withstand more complex in-hospital procedures as well. It is also an excellent option for patients with single level disease (for example L4-5 or L3-4) who meet certain criteria of spinal stenosis, or spinal slippage (spondylolisthesis).

Symptoms of a herniated disc include:
