For patients who have a protruding or herniated disc, open surgery is not always required for relief of pain. Provided there is ample disc height, DISC FX can be a highly effective option to eliminate back or leg pain.

Between each vertebra in the spine is a disc that serves as a shock absorber to protect the vertebrae. These discs have an outer layer (the annulus) and an inner section (the nucleus). The discs can be damaged because of injury or age.

If the annulus is torn, the nucleus can leak, leading to what’s known as a protruding or herniated disc. Up to 50% of low back pain can be due to damaged discs with tears or protrusions.

Rather than endure the rigors of major surgery, Advanced Pain Management & Spine Specialists offers qualified candidates DISC FX to restore comfort and mobility. In a 30-45 minute procedure, DISC FX is able to decompress the disc and seal tears in the annulus.

To determine if you are a candidate, you’ll begin with a physical exam and diagnostic tests (such as MRI or discography). While highly successful for qualified candidates, DISC-FX may not be beneficial for discs that are severely degenerated and have lost too much height.

A fairly common cause of Lumbar Spinal Stenosis (LSS) for older adults experiencing bone loss due to osteoporosis is deterioration of the vertebral structure.

Common symptoms of LSS are:
• Low back pain;
• Pain, weakness or numbness in the legs, calves or buttocks;
• Cramping in the calves with walking, requiring frequent short rests;
• Pain radiating into one or both thighs and legs, similar to sciatica;
• Pain relief when bending forward, sitting or lying down.

Fortunately, advanced options are available to relieve the symptoms associated with Lumbar Spinal Stenosis, without surgery.

MILD® (Minimally-Invasive Lumbar Decompression)
is an outpatient procedure that has provided long-term results for many APMSS patients. It relieves pressure in the spinal canal by removing excess ligament tissue.

The MILD procedure is quick (about 30 mins. for most patients) and performed through an incision that is less than an inch. It is also covered by Medicare.

MILD results are typically experienced within a few days, providing:
- Reduction in pain
- Restored mobility
- Increased standing time of up to an hour
- Increased walking distance (up to 4,000 ft)

The procedure is performed in the APMSS facility’s Park Center for Procedures. During this, the patient can relax in a “twilight sleep” state and return home within hours.

Sciatica occurs when one or more of the nerves that extend from the lower back and into the legs become irritated or inflamed. Fortunately, this often improves with time and self-care.

Self-care can take a few weeks or several months. Improvement may occur with rest, gentle movement, anti-inflammatory medication, stretches, and exercises to strengthen the core muscles. Lifestyle changes may also be needed to prevent reoccurrence.

Improvement at home starts with...
• Anti-inflammatory medications
• Use of cold packs first, switching to heat after a few days
• Over-the-counter pain relievers as needed
• Gentle stretches for the back and legs
• Strengthening the core muscles through careful exercise

Sciatica has a high recurrence rate (around 80%). When sciatic pain does not improve through at-home care, it may be due to underlying issues such as a herniated disc, spinal stenosis, or tumor.

Spinal stenosis can cause...

• Inability to stand for more than 10 - 15 minutes
• Buttock and leg pain that worsens with standing and walking
• Leg cramps, leg spasms, or “charlie horses” at night
• Choosing to sleep in a recliner or in a fetal position

Spinal fusion surgery in a hospital typically requires 3 - 4 hours followed by a 2 - 3 month recovery. This is where Minuteman, a minimally-invasive spine stabilizing device (fusion) is ideal for patients wishing to avoid open surgery and the lengthy recovery.

Minuteman takes about 30 - 40 minutes in our outpatient surgery center. It is performed through a one-inch incision and does not involve cutting any muscles or bone. Minuteman is especially beneficial for patients with single level disease (for example L4-5 or L3-4) who meet certain criteria of spinal stenosis, or spinal slippage (spondylolisthesis).

After surgery, leg pain should ease within days. Physical therapy will lead to increased walking and standing endurance as well as better balance and greater function.

RADIOFREQUENCY ABLATION

With age, the small joints in the rear of the spine degenerate or wear down as the soft tissues between the joints thins This causes the joints to rub together, leading to inflammation, stiffness and pain.

Could one injection relieve months (or years) of miserable pain?

In some cases, a correctly targeted injection provides significant relief, or even eliminates long-standing pain.
