Enrollment for Dysphagia Grand Rounds closes tonight. Before it does…here is one thing you can use tomorrow whether you enroll or not.

If you have ever struggled to justify a FEES referral to a resistant team, start here.

Three things I document every time:
1. The chart — recurrent pneumonia, unexplained weight loss, neurological history, prior intubation. That history is your foundation.
2. Bedside findings: wet vocal quality, multiple swallows per bolus, coughing during intake, oral residue. Document every single one specifically and objectively.
3. The mismatch: this is the one most SLPs miss. When the patient says they are fine but the objective picture tells a different story, document that discrepancy directly.
“Patient denies swallowing difficulty. However objective findings including wet vocal quality, recurrent pneumonia history, and unintentional weight loss are inconsistent with patient report and warrant instrumental assessment to rule out silent aspiration.”

What makes Dysphagia Grand Rounds different from everything else you’ve tried…

This video is one example of why we need FEES! This patient was placed on nectar at bedside with no instrumental. Pt admitted with CVA, coughing with thin liquids and SLP completed CSE and wrote a diet order for Soft and Bite Sized and Nectar.

It still shocks me every time this happens..
it shouldn’t because it is happening toooo much!

I don’t talk about this a lot but here’s why I created Dysphagia Grand Rounds.

Every day I see the benefits of FEES in my patients at the clinic in fort smith AND with patients and SLPs who I provide mobile fees for across acute care, inpatient rehab, LHATCs, SNFs and HH agencies.

Because of the privilege of being able to witness the systems and outcomes across a wide variety of settings and geographical locations across our region… I get to see the best and the worst of our dysphagia management in the every day interactions. ANNNDD recently I realized that one of the layers of the issue is that there is a shortage of radiologists in the country but especially within Arkansas. When ArkSHAs President came to me to help with Ready Set Scope, I was reminded about how much quality swallow studies were lacking across the region….not just in my area! It reminded me about I knew how hard it was for me in the beginning, about all the patients that I didn’t advocate for years ago and that we typically just need one person to believe in us and support our ideas to get to the next step in order to feel comfortable either picking up a scope ourselves and becoming an endoscopist or advocating for mobile FEES in the setting we work in. 💛Dysphagia Grand Rounds is May 27th @ 3 PM.

I’ve been seeing a lot of swallow studies that are incomplete. The problem is that acute care VFSS reports follow the patients for EVER.

The benefit of FEES is that we can take our time and trial all 👏the 👏things!

FEES has so many opportunities for biofeedback and education after the procedure!

Being able to accurately diagnose swallowing issues and train my patients with strategies based of live videos has been a game changer!

Gathering a support system prior to beginning.
And having a trusted guide to walk you through the maze of the FEES world.

Here's how to start applying this today:

Step 1: Line up more than one phone consult with a FEES competency mentor.
Step 2: Schedule one lunch/ coffee/ walk & talk with a potential referral source.
Step 3: Begin having one hard conversation with your family & friends about how much this journey means to you.

Being able to see what is actually causing your patients swallowing problem, knowing what recommendations to make, and becoming skilled at FEES report writing will not only change your patients life... but will 10X your confidence in yourself!

This is one skill set will make you feel like you can do anything you put your mind to. AND it will be the best financial investment you can make in your practice if you do it correctly!

If you feel like an imposter every time you think about adding a new clinical skill, that feeling is not a red flag. It is proof that you take your patients seriously.

If you’re a Med SLP who is holding back from pursuing FEES because imposter syndrome has convinced you that you are not ready, it’s more likely that you are worrying about getting it right and being the best therapist you can be and THAT makes you someone who is actually MORE equipped to do it well!

It sounds cliché, but being ready isn’t a feeling — it’s a choice. I’ve found that it’s best not to wait for the confidence. I’ve built it up by showing up anyway.
