⚖️ Important Update on Compounded Porcine (Desiccated) Thyroid Prescriptions ⚖️

We want to keep our patients and providers up to date on an FDA ruling that will affect compounded porcine thyroid products.

📌 Our manufacturers have informed us that porcine thyroid powder will no longer be available in about 12 months. After this time, it will be reclassified as a biologic drug—meaning it cannot be used in compounding.

➡️ Why the change?
Biologic drugs are medicines that originate from living cells (such as bacteria, yeast, or animal cells). In this case, porcine thyroid powder comes directly from a pig’s thyroid gland. This type of FDA reclassification is not new—HCG was moved to the biologic category back in 2019/2020, which impacted compounding in a similar way.

🙏 We know the headlines around this have caused concern, and we want to clarify some important points:

✨ This ruling applies to compounding pharmacies.
Large pharmaceutical companies can still manufacture porcine-based thyroid medications such as Armour Thyroid, Westhroid, and NP Thyroid. However, independent compounding pharmacies like ours will no longer be able to prepare custom porcine thyroid capsules, liquids, or other forms once the change takes effect.

✨ Bio-identical thyroid compounds ARE NOT impacted.
Bio-identical Levothyroxine (T4) and Liothyronine (T3) will continue to be available for compounding. These medications are not derived from animal cells.

💡 What this means for you:

- Porcine thyroid compounds can still be filled for the next ~12 months.
- After that, compounded porcine thyroid products will no longer be an option.
- Bio-identical thyroid options (T4 and T3) will remain available and unaffected.

We will keep you informed as the timeline becomes more defined, and we are here to answer your questions. Your care and continuity of treatment remain our priority.

1. Do Your Homework Ahead of Time

❓ Do you offer IV NAD+ infusions?
➡️ We don’t offer IV infusions of any kind here at Fort Wayne Custom Rx. However, if you’re interested, I recommend reaching out to Dorinda Mosbrucker, NP at The Scarlet Drip—they do a fantastic job with NAD+ infusions locally.

❓ What NAD+ compounding options do you have?
➡️ We’re currently in the early stages of developing a NAD+ nasal spray. If you’re interested, call and ask for me—I’ll be happy to add you to my list of patients to contact once it’s ready.
➡️ We’re also investigating the possibility of carrying a NAD+ injectable for subcutaneous (under the skin) use.

Do I need to take TMG alongside my NMN supplement❓

💊 Pharmacist Austin here! This is a great question, and I’m glad it came up after our NAD+ discussion.

When your body processes NMN, it uses methyl groups (think of these as small chemical helpers). Over time, frequent NMN use may increase your body’s need for methyl donors. That’s where TMG (Trimethylglycine) comes in—it helps replenish those methyl groups.

👉 Here’s a fun fact: Trimethylglycine gets its name because each glycine molecule has three methyl groups attached. Those methyl groups are exactly what your body can use to support healthy methylation when taking NMN.

👉 Do you have to take TMG with NMN?

Not necessarily. Some people do just fine with NMN alone.

But for long-term use, pairing NMN with TMG may be a smart idea, especially if your diet is low in other methyl donors like folate, B12, or choline.

I often recommend considering TMG as an add-on for patients who plan to be consistent with NMN supplementation.
