Regenerative Orthopedic Case of the Week: Stem cell therapy for knee arthritis STILL WORKING 7.5 years later!

Cecilia presented with bilateral knee arthritis pain in 2018. Pain with standing, bending, and squatting. X-rays at the time revealed moderate arthritis with joint space loss in the medial aspect of both knee joints with spurring. After discussing treatment options, she decided to have adipose stem cell treatment. Within three months, her pain had completely resolved. She has remained pain-free this entire time. I saw her today for a different issue. She remains pain-free. X-rays of her knees today revealed that her joint space narrowing has not advanced since her stem cell treatment. She has no pain with normal activities throughout the day. Her knee arthritis does not limit her activities at all.

Regenerative orthopedic case of the week: Shockwave treatment for persistent Achilles pain after surgical repair. 

Megan is a 36-year-old female who ruptured her Achilles tendon playing pickleball 4 months ago. She underwent surgical repair within a week. One month into her recovery, she developed an open wound over the incision secondary to suture rejection. Ultimately, she developed notable scar tissue in the region and loss of motion with increased pain. Despite physical therapy, she has had persistent loss of motion and pain with weight-bearing activities and continued to walk with a limp. Her surgeon referred the patient to consider shockwave treatment. Shockwave is a noninvasive treatment that stimulates blood flow, stem cell function and regenerative capability. Today, after four weekly sessions of shockwave treatment, her pain symptoms have improved 70%. Her range of motion is almost completely normal and she is now walking without limp.

Regenerative Orthopedic Case of the Week:

CG is a 54 year female avid pickleball player who plays 6 days a week. Four months ago, she developed gradually progressive right lateral elbow pain with play. Despite the pain, she has continued to play and the pain is increasing. Sharp and aching pain with grasping, twisting, lifting and especially pickleball. She has tried some physical therapy and dry needling without notable improvement. On exam, she has significant tenderness about the lateral elbow, pain with resisted extension of the wrist and twisting the forearm (supination). Xrays are negative. Ultrasound shows significant area of tendonitis with suspected tear. After discussing treatment options, she decided to have Platelet Rich Plasma (PRP) treatment today. During procedure, as I injected numbing medicine into the tendon, the suspected tear became obviously visible. You can actually see the tendon separation from the bone! These tears are often not visible until placed under injection pressure.

PRP is a very successful treatment for tennis elbow even with partial tears. Improvement does not happen over night. Tendon healing can take 10-12 weeks or longer. Given the significance of CG's injury, she may require a second PRP treatment. I do expect that she will be able to return to her previous level of pickleball without surgery.
