When leaders in our Wisconsin region decided to implement the Age-Friendly Care model two years ago, they decided to dream big. Instead of starting with one unit, they decided to roll out the patient-centered care model and its framework across the region's seven hospitals. Now, equipped with a dashboard for care teams and improved patient satisfaction scores, the model is expanding even further. Catholic Health World from The Catholic Health Association of the United States has the story.

For a seventh consecutive year, SSM Health St. Agnes Hospital has received the American Heart Association’s Get With The Guidelines® - Heart Failure Gold Plus quality achievement award for its commitment to improving outcomes for patients with heart failure, meaning reduced readmissions and more healthy days at home.

About 6.7 million U.S. adults are living with heart failure, a number that is expected to increase to more than eight million by 2030. Despite the name, heart failure doesn’t mean that the heart has stopped working - it means the heart is having a hard time pumping blood and oxygen throughout the body. While there’s no cure for heart failure, patients can live a quality life by working with their health care team to create and stick with a plan that may include medication, symptom monitoring, and lifestyle changes.

The Get With The Guidelines - Heart Failure quality achievement award is earned by hospitals that demonstrate a commitment to treating patients according to the most up-to-date guidelines as outlined by the American Heart Association. Get With The Guidelines puts the expertise of the American Heart Association to work for hospitals nationwide, helping ensure patient care is aligned with the latest research- and evidence-based guidelines. The program aims to increase healthy days at home and reduce hospital readmissions for heart failure patients. https://www.ssmhealth.com/employee-culture-communications/mynews/wisconsin/fond-du-lac/2025/8/heart-failure

Kaitlyn Zook, OTR/L, an SSM Health St. Agnes Hospital licensed occupational therapist, has been named the newest recipient of the BEE (Be Exceptional Everyday) Award.

Zook was recognized by two fellow teammates.

“Kaitlyn does an incredible job helping patients return home safely and confidently. She consistently goes above and beyond - whether that’s making sure patients have the right assistive devices or helping them solve their home setup based on descriptions and photos from family members. One example that stands out is when a patient’s family member sent photos and measurements of their shower setup. Kaitlyn took the time to closely consider the setup and worked with the patient to simulate it in the therapy gym. Her attention to detail and patient-centered approach made a huge difference in that person’s confidence and safety heading home. Kaitlyn’s dedication, compassion, and teamwork made her an invaluable part of our care team.”

“Kaitlyn is an extremely hard-working OT. She continuously goes above and beyond not only her patients, but also her co-workers. She is always willing to step in and provide a helping hand to those around her. She is someone I am confident about going to for questions, concerns, or advice when it comes to treating my own patients. She is well deserving of a BEE Award given all that she has done, and continues to do for her patients, and our department.”

The BEE Award is specifically designed to recognize non-nursing staff members who demonstrate great teamwork and communication skills, collaboration, professionalism, exemplary service, and a commitment to excellence.

On a sunny weekend in Fond du Lac, during the bustling member-guest golf tournament at South Hills Golf & Country Club, two young girls turned a simple backyard lemonade stand into a powerful tribute of love and generosity.

Olivia Schaeve and Lola Polacek, both Fond du Lac residents, hosted the stand in honor of Olivia’s grandmother, Susanne Stilwell, who passed away in December 2024.

“My mom was incredibly close to Olivia,” Rachel Schaeve, Olivia’s mother, shares. “As part of our grief journey, we’ve tried to keep her memory alive. The lemonade stand felt like a fun and meaningful way to do that.”

The girls, both age 9, embraced the challenge with enthusiasm. “They considered themselves quite the mixologists,” Rachel laughs. “They loved adding extra flavors and chatting with the golfers. As the donations started piling up, they set a goal to raise $1,000 - and they did it!”

The fundraiser brought in $1,035, all of which was donated to the Agnesian HealthCare Foundation to benefit SSM Health at Home Hospice, the care team who supported the Schaeve family during Susanne’s final weeks.

“When it became clear my mom wouldn’t recover, I reached out to SSM Health at Home Hospice,” Rachel recalls. “The weight lifted immediately. Kayla, her nurse, and Shiloh, her social worker, were compassionate, knowledgeable, and kind. Even the emergency line nurse on my mom’s last night was a source of strength.”

Thanks to the care provided by the hospice team, Susanne was able to pass with dignity, surrounded by love.

“To say we’re grateful is an understatement,” Rachel says. “This lemonade stand was more than just a fundraiser - it was a way for Olivia and Lola to give back to the people who helped us through the darkest time.”

The Agnesian HealthCare Foundation appreciates all donors who see the benefit of supporting the many services and programs offered by SSM Health. However, lemonade stands, like this example, not only provides a fun and educational experience but also help children develop a sense of responsibility and financial independence.

“By participating in a lemonade stand, children learn about budgeting, money management, and the importance of giving back to the community,” according to Mason Gravelle, Philanthropy Development Manager, Wisconsin Region, Agnesian HealthCare Foundation. “These experiences can lead to a lifelong commitment to philanthropy and charitable giving. Foundations often support organizations like Lemonade Day that help children learn these valuable lessons, ensuring that they carry these values into their later years. We truly appreciate Olivia honoring her grandmother, along with her friend, while helping other families receive vital hospice care for their loved ones.”

August 22 was Birds & Butterflies Day at SSM Health Adult Day Services, and one of the many cocoons the clients have been watching at the center hatched, which they released that morning. It flew away and then came back to a flower nearby.

Monarch butterflies are unfortunately on the 2025 endangered species list. In Mexico, monarchs represent the souls of departed loved ones, often returning in November which coincides with this important cultural observance.

Thank you to Sandy Isaac and Hannah Malson for donating the caterpillars and milkweed.
