Aesthetic medicine shouldn’t rely on assumptions, especially when we have the ability to actually see what’s happening beneath the skin.

Ultrasound allows us to visualize anatomy in real time:

• where previous filler has been placed

• where blood vessels are located

• how each patient’s tissue is uniquely structured

This adds a level of precision and safety that simply isn’t possible when treatments are performed blindly.

When you can see where you’re working, you can make more informed decisions, avoid critical structures, and tailor treatment to the individual in front of you - not a generalized approach.

Over time the midface loses structural support. When that happens, the cheeks flatten, shadows deepen, and the lower face can start to look heavier or more tired.

Instead of chasing individual lines, we restored support where it naturally diminishes first.

A small amount of carefully placed filler in the midface helped:
• restore contour
• soften shadowing
• improve overall facial balance

No dramatic change.

Okay… we lied. There are rules in this house. But we promise we still have fun 💉

If this is your first aesthetic appointment, those “rules” are actually what keep you safe and confident. They exist so we can properly assess your facial anatomy, talk through your goals and create a plan that feels thoughtful and never rushed or overdone.

Being a “cool injector” doesn’t mean saying yes to everything. It means explaining what makes sense for your face, starting conservatively, and sometimes recommending a phased approach or even waiting when that’s in your best interest.

We approach every face analytically and intuitively…studying structure, proportions, and the subtle imbalances that most people feel but can’t articulate. We don’t chase trends. We study anatomy, shadowing, projection, and vectors. At MHA, we treat with intention.

In this case, the imbalance was structural. Midface deflation and chin under-projection contributing to a heavier lower third.

Treatment strategy:
• Structural cheek support to restore anterior projection
• Chin augmentation to improve profile harmony
• Subtle lower face refinement to reduce heaviness

On profile, you’ll notice improved anterior cheek projection and a smoother lid–cheek transition. Frontal view shows narrowing and support of the lower third.
Oblique angle reveals a cleaner jawline and more defined facial architecture.

The temptation in aesthetics is more filler, more change, more drama. The industry rewards transformation. But restraint is power. Facial balancing requires discipline…knowing when to stop, when to support, and when not to touch.

Lip rehabilitation, not just lip filler. 👄

This case highlights using targeted ultrasound guided hyaluronidase use to dissolve migrated and irregular hyaluronic acid filler contributing to perioral heaviness, asymmetry, and blunted lip architecture.

The circled areas show regions where filler had migrated beyond the vermilion border and into the perioral tissue, affecting lip movement, contour, and definition. Strategic dissolution was performed to restore anatomy, soften tension, and reset the tissue.

Once inflammation resolved and the lips returned closer to baseline, the lips were re-treated conservatively, respecting natural lip anatomy, support zones, and balance between the upper and lower lip.

The result: improved contour, cleaner borders, better movement, and a more natural resting shape.

Lip rehab requires understanding anatomy, product behavior over time, and when less filler—or removal—is the right first step.

Dissolve. Reset. Rebuild.
