Dear patient,
I am writing to inform you that I will be leaving my current position at Chancellor Primary Care, effective January 1, 2026. This decision was difficult, but I am stepping away from the practice to pursue new opportunities. I am deeply grateful for your trust in me during your care.
I have compiled a list of medical offices that are taking new patients. Chancellor Primary Care will transfer your medical records to a new healthcare provider with a written request submitted to the office. We will ensure the records are sent securely, following HIPAA guidelines
If you have any questions or need further information, please feel free to contact me at 540-870-6550 before my departure. After that, you may reach the office directly at the same number.
