We are in the process of activating our new patient portal, which will allow you to request appointments and submit pharmacy refill requests. Please note that our previous portal is no longer active and cannot be used for any services.

Clinic will be closed 2/26 due to ongoing network outage. Our phone lines will be open for refill requests and concerns to be addressed with a provider. We do not have access to your health record at this time but can take note of scheduling requests. If you have a medical emergency please contact 911 or go to the ED for care.
