This week's challenge at Woodmont Center was "Guess how many cheese balls are in the container!". Our winner was a Director of Rehab Kelley! The total was 896 and Kelley guessed 922. 🎃🎃

Gertrude admitted to Woodmont for rehabilitation following a fall outside of the home that resulted in hip hemiarthroplasty. At initial evaluation, she required moderate assistance for lower body dressing, was minimal assistance for transfers and was ambulating up to 75 ft with 2WW with Min A.
Through her consistent participation in therapy for 3 weeks Gertrude made steady progress and successfully achieved her rehabilitation goals. At discharge, she demonstrates standby assist for lower body dressing, requires supervision for transfers and toileting, and is able to ambulate 150 feet with a rolling walker with Standby assist. She leaves Woodmont with increased strength, mobility, family support and confidence in her abilities.

Ms. Robinson is a long term resident at this facility and was picked up for therapy after a quarterly review. After several weeks of therapy she has shown improvements in her confidence, socialization, and overall motivation.💪🎃

We are proud to highlight the remarkable progress of LaVonne-she began her journey deconditioned, with difficulties getting out of bed and limited walking ability. Today, she has achieved independence with bed mobility and is now walking over 50 feet with confidence. Her self care was all moderate assistance and at discharge she was supervision for overall safety. —let’s celebrate this amazing achievement!💪❤️🐺

Sherri was admitted to Woodmont for rehabilitation following a femorofemoral artery bypass graft, poor wound healing and a recent episode of septic shock. At initial evaluation, she required maximum assistance for upper-body dressing, was dependent for transfers and toileting, and was unable to ambulate due to pain.
Through her consistent participation in therapy, Sherri made steady progress and successfully achieved her rehabilitation goals. At discharge, she demonstrates supervision-level independence for upper-body dressing, requires contact guard assistance for transfers and toileting, and is able to ambulate 100 feet with a rolling walker and CGA.💪

Sherri expressed excitement about her progress and return home, stating, “I can’t wait to get back home.” She leaves Woodmont with increased strength, safety awareness, and confidence in her abilities. ❤️
