Every three years, FHN, like all non-profit hospitals across the United States, conducts a Community Health Needs Assessment (CHNA) to identify the healthcare needs of the community they serve.
When the survey is completed, FHN will use the information to develop an action plan to address our communities' needs over the next three years.

Did you know that Medicare Part B covers a Wellness Visit with your healthcare provider every 12 months? Regular check-ins with your provider can help you and your provider address any concerns before they become a crisis.
If you've had Medicare Part B for more than 12 months, you can schedule an appointment right now - just call your provider's office! If you're new to Medicare Part B, a "Welcome to Medicare" visit which includes an initial preventative physical exam is covered at no cost.
If you don't have a primary healthcare provider, call us at 815-599-7060 (Monday - Thursday 7 a.m. - 6 p.m. and Friday 7 a.m. - 5 p.m.) and we'll help you find a provider before you hang up!
