When the CDC doesn't recommend a vaccine, physicians have to send a prescription to the pharmacy. We spent approximately 3-5 hours sending scripts to the pharmacy this week for boosters that were previously routinely available for patients WITHOUT A PRESCRIPTION. Governor Janet Mills saved us with a standing order so that all Mainers can get the COVID booster if they wish and we can get back to caring for patients.

We should have COVID boosters for children in the coming weeks but we must wait for the upcoming ACIP meeting on September 18th.

Starting January 1, 2026, patients can safely use their HSA to pay for Direct Primary Care (DPC).

The IRS clarified that DPC is not an insurance plan and therefore, can be funded with pretax HSA dollars. This means patients can spend less money on a high deductible health plan (HDHP) for the unexpected expenses and instead pay for a relationship with a physician that is personal and accessible.

There is more work to be done. We need to work on the cost of care in addition to the access. Businesses across the country recognize DPC as a foundational step to keep healthcare costs consistently low and avoid their employees having to use emergency rooms as primary care. Next, we need to help the millions of patients that will lose their Medicaid or insurance subsidies. Healthcare doesn't need to be this costly. Physicians need to step up and fight corporate medicine and improve access to care.

We no longer accept patients with typical HMO plans. These are designed for corporate insurance based care and we are not that.
Consider this when you are planning for your healthcare costs during open enrollment.

So what does this this mean for Americans?

1.) Starting next January, you can use your own Health Savings Account (HSA) to fund an affordable monthly agreement of $150 per month or less with your primary care doctor without worrying about the IRS knocking at your door because you are considered ineligible to fund an .

2.) Your employer may cover a agreement as a part of your health benefits plan, with even if that plan is a high deductible health plan (HDHP) with an HSA.

3.) Spending on that DPC agreement would count toward the deductible in your high deductible plan.

4.)Employers may offer these DPC plans with zero cost sharing and no out of pocket costs for primary care.

5.) Employers, led by the Purchaser Business Group on Health (PBGH), The ERISA Industry Committee Boeing Amazon and other businesses who have paved the way will now be able to offer a DPC benefit to ALL their employees.

6.) The federal government, for the purposes of both the tax code and the U.S. health code has acknowledged that DPC is an advanced primary care payment and delivery model, and that DPC IS NOT INSURANCE OR A HEALTH PLAN.

Telemedicine is wonderful when a physician has a long term relationship with their patient. Otherwise, online medicine is simply a dangerous vending machine for medicine/testing.
