Why we will not accept patients with Medicare Advantage.

Short answer: It is too much administrative work for our office.

Long answer: It is not health insurance provided directly from the government. It is a highly restrictive private health insurance masquerading as Medicare with a highly restrictive network that puts limitations on care and adds unnecessary hoops for practices to jump through.

Hopefully, if I get a chance to choose a Medicare plan, I will be choosing the following:
Traditional Medicare + Medigap Supplemental Insurance + Part D= 100% coverage

There are a lot of brokers that will offer help. Brokers are paid a commission and it might be higher for Medicare Advantage plans.

Insurances are flagging and blocking payment to physicians that spend more time with patients. This is why we don't participate with most insurances. We refuse to give ineffective care and waste the time of the physician and patient.

Most of our visits are higher level care defined by time or amount of body systems discussed. Can you imagine participating in a system where we force people to take time out of their day to come in (because Telehealth isn't always allowed) to talk to them about one problem for 15 minutes? This is ineffective and wasteful.

Prior authorizations were once a narrow tool to contain costs. Downcoding could be next.

When the CDC doesn't recommend a vaccine, physicians have to send a prescription to the pharmacy. We spent approximately 3-5 hours sending scripts to the pharmacy this week for boosters that were previously routinely available for patients WITHOUT A PRESCRIPTION. Governor Janet Mills saved us with a standing order so that all Mainers can get the COVID booster if they wish and we can get back to caring for patients.

We should have COVID boosters for children in the coming weeks but we must wait for the upcoming ACIP meeting on September 18th.
