We are a small, home-like, 40-bed skilled nursing center in Fremont, Ohio. We offer short-term stays, long-term care, rehabilitation care, respite care, and hospice care. Our skilled and experienced short-term rehabilitation team offers a wide array of therapies to help you regain function and safely return home or to the next level of care as quickly as your recovery allows.

Spending time together, checking in, and sharing simple moments can help create more good days for all of us. This month, we are focusing on those small, meaningful moments that bring comfort, connection, and a little more joy. Our Administrator and Admissions Director treated a few residents to a sweet little outing to Dairy Queen last week and it did not disappoint! We love when our team goes the extra mile to make our residents smile. 💛🎉
