Elevated Community Health is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental, and behavioral health services through a team-based model of care. alth services through a team-based model of care. We also accept Medicaid, Medicare and private insurance. The Care Clinic works with patients to make sure their health needs are met first and foremost. The Care Clinic knows that healthy people are able to stay in work, provide for their families, and contribute to our local community. Service fees for all patients without insurance are based on income and family size. Patients without insurance will be charged according to a sliding fee scale—based on Federal Poverty Guidelines—if deemed income eligible. No patient will be denied service for their inability to pay.

Noticing changes in memory or impacts on your daily routines can feel overwhelming, but you do not have to navigate it alone. Changes in memory, mood, thinking abilities, or daily habits are worth talking about with a health care provider and a trusted loved one. Early detection and diagnosis of dementia can help individuals and families better understand what’s going on, explore support options, and plan next steps together. Not every change means Alzheimer’s disease or dementia (ADRD). A conversation can be a great first step.

Join us at the ADRD Community Event on Thursday, May 28th at 2:30pm at the Summit County Community & Senior Center to learn more about Alzheimer’s disease and related dementias, local resources, and support for caregivers and families. This event is supported by funding from the Colorado Department of Public Health and Environment.

Small steps can support brain health at every age.

Some risk factors for Alzheimer’s disease and related dementias (ADRD), like age and family history, cannot be changed, but there are meaningful ways to support long-term brain health:

● Challenge your mind by trying new activities and learning new skills.

● Keep moving with exercise and physical activity to support a healthy heart and brain.

● Protect your head by wearing helmets and seatbelts.

● Be smoke-free and reduce the consumption of alcohol.

● Manage blood pressure and diabetes with healthy habits, regular care, and medications when needed.

● Eat balanced meals, maintain a healthy weight, and make quality sleep a priority.

Join Elevated Community Health for an ADRD education event at the Summit County Community & Senior Center on Thursday, May 28 at 2:30pm. Mental health support can make a difference at every age. This Mental Health Awareness Month, we’re recognizing the importance of care that meets kids where they are.

For elementary-aged children, especially those who have experienced trauma, support may look different than it does for adults. Sometimes, it looks like play. Through play therapy, children can express feelings, build trust, and feel safe sharing what they may not yet have words for.

Yirka Platt, one of Elevated Community Health’s School-Based Health Center behavioral health providers, reminds us that even when visits are not frequent, the time spent with a child can still matter deeply.

Mental health care is care. And for students, having support at school can make it easier to get help when they need it.

May is Mental Health Awareness Month, and at Elevated Community Health, we believe taking care of your mind is just as important as taking care of your body.

One in five U.S. adults experiences a mental health condition each year, and only about half get the care they need. We want to change that for our community.

ECH offers behavioral health counseling for ages 12 and up, in English and Spanish, in person, by telehealth, and at our school based health centers across Summit, Park, and Lake Counties.
