Elevated Community Health We proudly offer healthcare services to the medically underserved, uninsured, or underinsured at affordable costs.

Elevated Community Health is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental, and behavioral health services through a team-based model of care. Summit Community Care Clinic is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental and behavioral health services through a team-based model of care. We also accept Medicaid, Medicare and private insurance. The Care Clinic works with patients to make sure their health needs are met first and foremost. The Care Clinic knows that healthy people are able to stay in work, provide for their families, and contribute to our local community. Service fees for all patients without insurance are based on income and family size. Patients without insurance will be charged according to a sliding fee scale—based on Federal Poverty Guidelines—if deemed income eligible. No patient will be denied service for their inability to pay.

"You have to be in crisis to go to therapy.”

Not true.

You don’t need to wait until everything feels like too much. Therapy is not just for when things are falling apart. It's for building resilience, processing everyday stress, and checking in on your mental and emotional well-being.

Just like you visit your doctor for a yearly physical, you can visit a therapist for a mental health check-in. Sometimes it helps just to talk it out. Other times, it helps to learn a new skill or way of thinking.

Whatever you're going through, you don't have to go through it alone.

Walk-in vaccines are available every Thursday in September at our Frisco location. Most insurances cover flu shots, and affordable options are available for all ages.

The number 2 may seem small, but when it comes to your child’s smile, it makes a BIG difference!

- 2 dental visits each year help catch problems early and keep teeth strong.

- Brush 2 times a day for 2 minutes to fight cavities and protect that smile.
