We proudly offer healthcare services to the medically underserved, uninsured, or underinsured at affordable costs.

Elevated Community Health is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental, and behavioral health services through a team-based model of care. Summit Community Care Clinic is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental and behavioral health services through a team-based model of care. We also accept Medicaid, Medicare and private insurance. The Care Clinic works with patients to make sure their health needs are met first and foremost. The Care Clinic knows that healthy people are able to stay in work, provide for their families, and contribute to our local community. Service fees for all patients without insurance are based on income and family size. Patients without insurance will be charged according to a sliding fee scale—based on Federal Poverty Guidelines—if deemed income eligible. No patient will be denied service for their inability to pay.

🎉 Big news for our Women's Wellness Connection team!

Elevated Community Health has been selected as a Shirley Hass Schuett Quality Award winner by the Colorado Department of Public Health and Environment (CDPHE) through the Women's Wellness Connection program for FY 2024-2025!

We were recognized as a top performer in reaching women rarely or never screened for cervical cancer, completing follow-up for abnormal cervical and breast screenings, and reducing time from screening to diagnosis.

We were also honored as most improved in cervical cancer outreach, abnormal cervical screening follow-up, and data entry timeliness.

This award reflects the heart and dedication of our CPED-WWC team. Thank you for the incredible work you do for the people of Colorado. 💙

From checkups and dental care to behavioral health and school-based services, public health is about making care easier to access and easier to trust.

This year’s National Public Health Week theme is “Ready. Set. Action!” and it is a great reminder that strong communities are built through prevention, connection, and care that meets people where they are.
