CCACC Health Center 泛亞門診 CCACC Health provides primary care, preventive care, health-related services, and welfare applications for uninsured low-income residents of Montgomery County.

Community health clinic, serving Montgomery County, MD since 2003. Services:
- Primary Care
Diagnosis and treatment of acute and chronic illnesses (i.e. diabetes, hypertension, and asthma).
- Referral Services:
Patient referrals to connected facilities for diagnostic imaging at a discounted rate. Patient referrals to specialists and health management plans for follow-up care.
- Case Management
- Education and Outreach

Requirements:
- Montgomery County resident
- Uninsured
- Income below 250% Federal poverty level
- Age 18+

Bilingual Staff (Chinese).

Here are some ways to support your emotional well-being:

情緒健康不只是避免壓力,更是建立有助於您茁壯成長的生活習慣。

📓 Try expressive writing – Set a timer for 10-15 minutes and write freely about your thoughts or emotions. It’s about release.

🧩 Do something slightly challenging – Solving a puzzle, learning a new skill, or tackling a small project can boost confidence and mental clarity.

Caring for someone with Alzheimer’s can be challenging, but small strategies can make a big difference:

Silent risks start with silent numbers.🩸It’s National Glucose Awareness Week - a time to raise awareness about blood sugar health, empower our community, and promote habits that prevent and manage diabetes.

More than 1 in 3 adults in the U.S. has prediabetes, and most don’t know it. The good news? Prediabetes can be reversed. Whether you're managing diabetes or working to prevent it, here are some tips to support your journey:

美國每三位成人中就有一位患有糖尿病前期,但大多數人並不知情。好消息是:糖尿病前期是可逆轉的!
