🔪 A1 Pulley Release – Trigger Finger Surgery
This procedure is used to treat Trigger Finger (stenosing tenosynovitis)—a condition where the finger gets stuck in a bent position due to inflammation or narrowing in the tendon sheath.
👉 In the image above, a scalpel is shown cutting the A1 pulley, which is a part of the tendon sheath located at the base of the finger. This pulley normally helps keep the flexor tendon close to the bone as the finger bends and straightens.
💥 When the A1 pulley becomes too tight or inflamed, the tendon can’t glide smoothly—causing clicking, pain, or locking.
✅ By releasing (cutting) the A1 pulley, the tendon is freed to glide smoothly again, relieving symptoms and restoring function.

This surgical procedure involves cutting the transverse carpal ligament to relieve pressure on the median nerve — the one responsible for sensation in your thumb, index, middle, and part of your ring finger.
🔹 Why?
When this nerve is compressed, it can lead to pain, tingling, weakness, and numbness (aka Carpal Tunnel Syndrome).

🎿 Skier’s Thumb?
It’s not just for skiers! This injury happens when the thumb is forcefully pulled away from the hand — often tearing the ulnar collateral ligament (UCL) at the base of the thumb.
👉 Common causes: falls while skiing, sports injuries, or even aggressive thumb yanks!
💢 Symptoms: pain, swelling, and difficulty gripping.
🛠 Treatment: may include a splint, rest, or surgical repair (like UCL reconstruction) — followed by hand therapy to regain strength and motion!

🌀 TFCC Repair: What Is It?
The Triangular Fibrocartilage Complex (TFCC) sits on the pinky side of your wrist and acts like a shock absorber + stabilizer. When it’s torn (from falls, sports, or repetitive motion), it can cause pain, clicking, and instability.
🔧 A TFCC Repair involves stitching or anchoring the tear back in place—often done arthroscopically.
🖐️ Post-op? That’s where hand therapy steps in! Regaining motion, strength, and stability is key to getting back to daily life.

🛠️ EXTERNAL FIXATION
Sometimes a fracture is too unstable to fix with a cast or even surgery alone — that’s when external fixation steps in.
🦴 What is it?
External fixation is a surgical method where metal pins and rods are placed outside the body to hold the bones in position. It’s commonly used for severe wrist or forearm fractures, especially when there’s swelling, open wounds, or multiple fractures.
🔧 During Surgery
* Pins are inserted into the bone above and below the fracture.
* These pins are connected to a stabilizing frame outside the skin.
* It’s quick, effective, and minimally invasive.
🩹 Post-op Recovery
* The frame stays on for 6–8 weeks, depending on healing.
* Wound care is crucial to prevent infection around pin sites.
* You may not be able to lift or bear weight with the limb right away.
* Hand therapy begins early to keep nearby joints mobile and prevent stiffness in fingers, elbow, and shoulder.
* Once the frame is removed, we work on regaining motion, strength, and daily function.
💡 Therapy Tip:
Even while the fixator is on, motion matters! Don’t let the rest of your arm get stiff — we guide you through it step by step.
