Regular check-ins with your primary care provider are a great way to stay up-to-date with your provider, and give them updates about what’s been going on in your life and any changes to your goals. Keeping up with your provider will allow them to tailor their recommendations to your specific situation and get you better care.

In your wellness visit, Polly will spend the time to get to know you, and get a complete picture of your health so she can connect dots and get you help quickly when you need it.

And we're live! Integrate is soft opening this week, and accepting new patients who are covered by Medicare, BCBSKS and United Healthcare commercial plans.

Exciting news, SWKS!

Integrate Family Health is now participating in-network with United Healthcare Commercial plans, BCBS of KS, and Medicare of Kansas!

As a reminder, Integrate is planning to accept all insurances, including all 3 Medicaid plans: United Healthcare Community, Healthy Blue, and Sunflower Health. If you'd like us to accept your specific insurance, and don't see it in the list above, drop a comment below and let us know!

Introducing Integrate Family Health and Polly Elpers, Family Nurse Practitioner!

Integrate is Garden City’s new, independent primary care provider, open to all and accepting all major insurers, including Medicaid. The practice will be focused on listening to and learning about each patient, caring for them and all their health needs, and providing education and coordinating care so that every patient leaves with a better understanding of how their body works, how it needs to work for them to live the life they want, and what their next steps are for getting that care.
