We will not do a PA for weight loss medication anymore.

If insurance does not cover it, you will need to pay out of pocket or see a weight loss clinic.

Most “weight loss” medications are on a formulary list for specific diagnosis and weight loss is not one that will be covered. There are certain criteria that must be meet and weight loss is not in the criteria.

Example: Ozempic, mounjaro are formulary approved for diabetes, not weight loss, not sleep apnea.
In these cases, we have to provide labs, diagnosis and chart notes.
