If your practice participates in Community Care Physician Network, you are included in the NC State Health Plan’s Preferred Primary Care Network, effective Jan. 1, 2026.

Participation brings meaningful benefits for both practices and patients, including enhanced quarterly payments, lower patient copays, reduced prior authorizations, and the ability to bill multiple services per visit.

Please remember that Aetna administers the State Health Plan. Providers must be enrolled in the Aetna POS II network to participate. Practices should confirm enrollment if they have not already done so.

The CCPN board of managers has instituted several updates to CCNC’s Medicaid Prospective Quality Payment methodology that create new revenue opportunities for practices.

CCNC will now issue advanced payments for care gap closures as they occur and add incentive payments for prompt follow-up care after high-priority ED and inpatient discharges. These changes are designed to support high-risk patients, reduce avoidable utilization, and strengthen shared savings performance.
