How can predictive modeling help reduce repeat emergency department visits?

Two new data briefs from Community Care of North Carolina highlight how CCNC is using data to strengthen care management and better identify patients who may benefit from additional support after an ED visit.

Instead of focusing only on a narrow set of “avoidable” diagnoses, CCNC developed an ED Risk Score that identifies patients most likely to return to the ED. This allows care managers to focus outreach where it can make the greatest difference.

Among patients in the highest risk category, CCNC care managers prevented one ED visit for every five patients who received care management support.
