Modern Vue is searching for a Registered Nurse who will coordinate all aspects of the client's home care and interacts with other disciplines as needed. He/she provides professional care to the client, as indicated by Agency policies, State and Federal laws/ regulations. In-home admission visits and eligibility determinations are part of the RN' duties, as is the development of the Care Plan.

The RN will also prepare paperwork necessary for Agency policies and federal/state laws and assists with the training and supervision of Companions.

Follows Agency public relation policies regarding outside speaking and professional affiliations and clears all related requests with Agency Manager/Administrator.

Assumes other duties, as requested by the Agency Director/Administrator or Governing Board.

We are searching for CNA's
to work at our Home Health Agency.
Our Certified nursing assistants help individuals who have functional, physical, or mental impairments
accomplish their daily activities. You will provide hands-on care such as Bathing, Dressing, Toileting, Feeding
and Ambulation as described in patient’s plan of care and physician orders.

Responsibilities/Activities:

Perform home management skills and hands-on care services under the supervision of clinical nurse

Ability to understand plan of care
and physician orders to deliver care appropriately.

Follow through with principles and processes for providing client services, including needs determinants, meeting quality standards and evaluation of client satisfaction.

Deliver emergency interventions within scope of practice including emergency first aid and CPR.

Maintaining records and completing documentation on time.

Effectively communicate with Supervisor and co-workers.

Attend orientation, in-service training sessions and staff meetings.

Work assigned schedule Monday
to Friday, Weekends required, Day shift, Night shift, Overtime or on-call shifts (to be determined)
